Resolution No. PC-2003-16 <br />Page 3 <br />EXHIBIT "B" <br />CONDITIONS OF APPROVAL <br />Case PCUP-82 <br />(Sharon Carrell -Ladies Workout Express) <br />The location and operation of the proposed use shall conform substantially to <br />Exhibit "A" (the site plan, floor plans, and details of operation), dated <br />"Received, Apri14, 2003", on file with the Planning Department, except as <br />modified by the following. <br />The operation of the ladies fitness/weight loss facility shall be limited to the <br />following hours: <br />Monday -Friday: 6:00 a.m. - 8:00 p.m. <br />Saturday: 8:00 a.m. -11:00 a.m. <br />Sunday: Closed <br />If additional hours of operation or activities beyond what was included in the <br />applicant's statement of operation, dated "Received, April 4, 2003", on file in <br />the Planning Department, are desired, prior City review and approval is <br />required. Such modifications may be approved by the Planning Director if <br />found to be in substantial conformance with this approval. The Planning <br />Director may also refer the matter to the Planning Commission if the proposed <br />changes would be significant. <br />4. If operation of this use permit results in conflicts pertaining to parking, <br />interior or exterior noise, traffic/circulation, or other factors, at the discretion <br />of the Planning Director, this conditional use permit may be submitted to the <br />Planning Commission for their subsequent review at a public hearing. If <br />necessary, the Planning Commission may modify or add conditions of <br />approval to mitigate such impacts, or may revoke the said conditional use <br />permit approval. Possible mitigation measures may include, but are not <br />limited to, reducing the number of exercise equipments, modifying the hours <br />of operation, or other measures deemed necessary by the Planning Director. <br />5. The applicant shall provide address and unit numerals on both the front and <br />rear doors. The applicant shall provide knox box keys for both doors. <br />6. The applicant shall place one 2A lOB;C fire extinguisher near the back door <br />in plain view. <br />This use permit approval does not include approval for signage. Proposed <br />signs shall be consistent with sign program for Oak Hills Shopping Center, <br />and are subject to the approval by the Planning Director prior to installing any <br />signs. <br />[end] <br />
