8/6/2019 <br />HOPE Outreach <br />The HOPE Project works with a challenging population that is defined by its complex barriers to housing, <br />varying from undiagnosed mental health issues to criminal records. Despite these challenges, we have <br />managed to successfully engage in outreach with more individuals that our contracted requirements. <br />24. For CAPER: Describe any problems or delays encountered with the project or program. How <br />were they handled? What effects, if any, were there on costs? Describe any changes that made the <br />project or program successful now or in the future. <br />Report 1 <br />n/a <br />Report 2 <br />A major challenge in the HOPE Project is maintaining engagement in our population due to the long time it <br />takes to house individuals. Very little affordable housing stock is available in the Tri -Valley area for <br />Pleasanton residents with very low income, even less so than what we experience in our other jurisdictions <br />in Alameda County. These frequently lead to poor morale with our participants and challenges motivating <br />them to complete important tasks for case management purposes. In these situations, we internally consult <br />to implement creative housing solutions, such as relocation to family members in other states, and <br />additionally do clinical consultations when challenges in the housing market have ramifications for their <br />mental health. We will continue to track the extent of these issues, such as the realistic timeline in which it <br />takes for individuals to be housed given available resources, and work on our data reporting to provide <br />advocacy at a City and County level to better serve this population. <br />25.
