Funds would <br /> be used to hire a Spanish-speaking clinician to serve <br /> residents of Dublin,Livermore,and Pleasanton. <br /> Child Care Links— Program provides free diapers, children's books,bilingual $5,500 Gen Fund <br /> Early Literacy Diaper Pantry early literacy materials,and car seats to low-income <br /> families living in Pleasanton and Tri-Valley Community. <br /> City of Pleasanton— HUD-restricted 20%maximum cap for City staff to $63,758 CDBG <br /> CDBG Program Administration administer the CDBG program. Admin <br /> City of Pleasanton— Required annual payment on the HUD Section 108 loan $23,184 CDBG <br /> HUD Section 108 Loan Repayment that was obtained in 2015 to provide funding for the Axis Capital <br /> Community Health medical clinic project. <br /> CityServe of the Tri-Valley— Agency coordinates a wide range of community $19,000 Gen Fund <br /> Crisis Intervention/Sheltered resources(services,housing resources,etc.)by <br /> Residents mobilizing volunteers working in collaboration with local <br /> organizations. <br /> CityServe of the Tri-Valley— Funding will be used for the facility build out for its main $53,024 CDBG <br /> CDBG Grant Facility Build Out office and primary staff location to be at the Multi-Service Capital <br /> Center located in Livermore. The facility is owned by the <br /> City of Livermore. <br /> Easter Seals Bay Area Kaleidoscope Teen Group;education and socialization $5,000 Gen Fund <br /> program for disabled young adults aged 18-22 <br /> transitioning from high school including community-based <br /> volunteer,employment,recreation, leisure,and <br /> educational opportunities. <br /> Hope Hospice Grief Support Center provides grief support services to $8,400 Gen Fund <br /> Pleasanton families coping with loss and end of life <br /> issues. <br /> Legal Assistance for Seniors Provision of free legal advice and representation and $6,500 Gen Fund <br /> community education presentations in the areas of elder <br /> abuse,guardianship of minor children,health law,public <br /> benefits,and legal services for immigrants to Pleasanton <br /> seniors. <br /> NAMI Tri-Valley NAMI Connection is a recovery support group program $4,000 Gen Fund <br /> that offers respect, understanding,encouragement,and <br /> hope. The weekly 90-minute support group are led by <br /> trained facilitators living in recovery themselves. <br /> Open Heart Kitchen— Pleasanton's hot meal program and children's weekend $13,400 Gen Fund <br /> Hot Meal&Weekend Bag Lunch box lunch program provides hot meals to families and <br /> weekend lunch at seven Pleasanton school sites. <br /> Open Heart Kitchen— Pleasanton Congregate Meal Program provides nutritious $47,818 CDBG <br /> Senior Meal Program meals to low income seniors at the Pleasanton Senior Public <br /> Center and Ridge View Commons. Services <br /> Spectrum Community Services Provision of home-delivered meals to homebound $13,900 Gen Fund <br /> Pleasanton seniors on three routes through the Meals on <br /> Wheels program. <br /> Sunflower Hill Expansion of horticulture classes to additional schools, $12,000 Gen Fund <br /> transition programs and adult day programs in the Tri- <br /> Valley area,adding improvements via a more formal <br /> education curriculum to meet the needs of the special <br /> day classes and individuals. <br /> Page 5 of 11 <br />
