The City reserves the right to meet and confer with the Union for the purposes of coordinating <br /> retired employee's hospital and medical care in the event Federal or State legislation is <br /> implemented that effects retiree's medical insurance. <br /> 16.3 Dental <br /> The City shall contribute an amount necessary to provide dental care benefits for the individual <br /> employees and eligible dependents. The City shall provide a lifetime maximum orthodontia coverage of <br /> $2,000 for individual employees and eligible dependents. <br /> 16.4 Life Insurance <br /> The City shall contribute an amount necessary to provide $50,000 life and $50,000 accidental death and <br /> dismemberment insurance coverage for the-individual full-time employees. <br /> Employees may purchase, at their employees own expense and within the limits set by <br /> the insurance carrier,,—additional amounts of life insurance under the existing policy. <br /> 16.5 Vision Care <br /> A vision care plan will be provided as an option for employees and the cost of such coverage shall be <br /> included in the 125 Plan(see Section 16.6). <br /> 16.6 Cafeteria Plan <br /> An Internal Revenue Code Section 125 Cafeteria Plan is provided to each full-time employee who is <br /> eligible to enroll in one of the medical insurance plans offered by the City : ' -• ..- . • . • •- - •- <br /> but elects to waive participation in such medical plans and/or the City's vision care <br /> plan. Proof of alternative medical coverage is required for an employee to waive participation in the <br /> City's medical plans. <br /> The 125 Cafeteria Plan . . •. . . - ' - . - . . • • . . - • . . - <br /> affords employees an the opportunity to obtain plan credits for the purpose of: (1) purchasing <br /> supplemental benefits; (2) redirecting credits to a flexible spending account pursuant to IRC 125; or (3) <br /> cashing out credits at a rate of$0.35 on the dollar. The one tier credit formula is Plan credits are based <br /> on the combined sum of the monthly premiums for Kaiser employee-only medical coverage and VSP <br /> "employee only" premium the vision care plan at family level coverage; adjusted by the City, annually <br /> on a dollar-for-dollar for annual rate increases basis to match any changes in the monthly premiums. <br /> •- • ., . , ., - . .' . . .. - - .• . -- .
