EXHIBIT A <br />CONDITIONS OF APPROVAL <br />PUD -84-21-04M, Braddock and Logan <br />Civic Square Apartment <br />4800 Bernal Avenue, <br />March 25, 2009 <br />Project Specific Conditions <br />The 12 -unit, 3 -building addition and remodeling including site changes, <br />landscaping, buildings, etc., covered by this approval shall be constructed <br />substantially as shown on the development plans, Exhibit B, on file with the <br />Planning Division, except as modified by the following conditions. Exhibit B for <br />PUD -84-21-04M, dated "Received March 11, 2009" includes: "Administration/ <br />Clubhouse Building Floor Plans and Elevations", "Apartment Building Floor Plans <br />and Elevations', "Focused Site, Landscaping, and Tree Removal Plans", "Overall <br />Site Plan", and "Materials and Color Samples". Minor changes to the <br />development plan may be allowed subject to the approval of the Planning <br />Division if found to be in substantial conformance to the approved exhibits. <br />2. All conditions of approval for this development plan shall be reprinted and <br />included as a plan sheet(s) with the building permit plan check sets including <br />grading, construction, and landscaping plans, which shall be kept on the project <br />site at all times. The applicant may request these conditions of approval be <br />attached to the plan set as a separate document which he/she is responsible for <br />obtaining and ensuring the conditions are securely attached. Prior approval from <br />the Planning Division is required before any changes are constituted in site <br />design, grading, etc. It is the responsibility of the project developer to ensure that <br />the project contractors are aware of, and abide by, the approved plans and all <br />conditions of approval. <br />3. The applicant shall reroof an existing mailbox structure and repair the chain link <br />fencing on the south property line adjoining the rail corridor before the first final <br />inspection. <br />Construction and Demolition for all Proiects <br />4. The applicant shall submit a waste management plan to the Building and Safety <br />Division prior to issuance of building or demolition permits. The plan shall <br />include the estimated composition and quantities of waste to be generated and <br />how the project developer intends to recycle at least 75 percent of the total job <br />site construction and demolition waste measured by weight or volume. Proof of <br />compliance shall be provided to the Chief Building Official prior to the issuance of <br />a final building permit. During demolition and construction, the project developer <br />
