<br />location, height, and design of each existing and planned personal wireless service facility <br />within the City limits and within one-half mile therefrom. <br /> <br />15. A statement of intent whether the facility would be co-located. <br /> <br />16. A letter which states the personal wireless service provider's commitment to <br />allow other personal wireless service providers to co-locate antennas on their proposed <br />facilities wherever structurally and technically feasible, and to provide at any time <br />additional information, as requested by the Zoning Administrator, to aid in determining <br />whether or not another personal wireless service provider could co-locate on/near their <br />facilities if approved. <br /> <br />17. A letter stating: 1) the power rating for all antennas and back-up equipment <br />proposed, 2) that the system, including the antennas, and associated equipment <br />cabinets/structures, conforms to the radio-frequency radiation emission standards adopted <br />by the Federal Communications Commission, including operating within its frequency <br />assigned by the Federal Communications Commission, and 3) that operation of the <br />facilities in addition to ambient radio frequency emission levels will not exceed adopted <br />Federal Communications Commission standards. <br /> <br />IS. A letter stating that the proposed personal wireless service facility shall be <br />operated in a manner which complies with the Federal Communications Conunission's <br />regulations regarding signal interference. <br /> <br />19. Reference to any easements necessary. <br /> <br />20. All proposed signage, including emergency signage as required by Subsection <br />18.11 0.160.8. <br /> <br />B. Additional information as deemed necessary by the Zoning Administrator <br />which may include, but is not limited to, the following: <br /> <br />I. Information sufficient to determine that the personal wireless service provider <br />has applied for and received all applicable operating license or other approvals required <br />by the Federal Conununications Commission and California Public Utilities Commission <br />to provide personal wireless services within the City. <br /> <br />2. The types and range of sizes of antennas and equipment cabinets which could <br />serve as alternatives for use by the personal wireless service provider. <br /> <br />3. A USGS Topographic map or survey, to scale, with existing topographic <br />contours showing the proposed antennas, accessory structures, and new roads in an area. <br /> <br />4.
