Funds <br /> Open Heart Kitchen Pleasanton Congregate Meal Program provides nutritious $47,500 CDBG/ <br /> meals to low income seniors at the Pleasanton Senior Center Gen Fund <br /> and Ridge View Commons. <br /> Pleasanton Unified School Through existing PUSD nurses,coordinates access to local $5,000 Gen Fund <br /> District dentists,optometrists,and other medical service providers for <br /> low-income,uninsured,or under-insured school age children in <br /> the PUSD system. <br /> Pleasanton Unified School College Preparation and Readiness(CPR)program focuses on $0 --- <br /> District supporting transition-aged kinship and foster youth in the areas <br /> of student engagement and postsecondary education options. <br /> The HSC did not recommend a second year of funding for this <br /> program.They felt that the program was a better aligned with <br /> the Youth Community Grant program. <br /> Sandra J.Wing Healing Provides financial assistance for complimentary therapies for $0 Gen Fund <br /> Therapies Foundation cancer patients undergoing chemotherapy or radiation. Notes <br /> from the discussion of this item are included in the HSC <br /> Minutes. In summary,the HSC did not recommend funding due <br /> to their FY17/18 focus on prioritizing safety net services for low <br /> income residents. <br /> Spectrum Community Services Provision of home-delivered meals to homebound Pleasanton $8,000 Gen Fund <br /> seniors on three routes through the Meals on Wheels program. <br /> Sunflower Hill Capital garden operations equipment for Sunflower Hill $0 --- <br /> Gardens at Hagemann Ranch,a 1-acre farm designed to <br /> provide educational and vocational horticultural opportunities to <br /> adult individuals with developmental disabilities from several <br /> groups/programs in the Tri-Valley area.The HSC did not <br /> recommend funding of this program as the purchase of the <br /> equipment would not directly support the delivery of safety net <br /> services.The HSC did recommend funding the other grant <br /> application from Sunflower Hill. <br /> Sunflower Hill Expansion of horticulture classes to additional schools, $5,000 Gen Fund <br /> transition programs and adult day programs in the Tri-Valley <br /> area,adding improvements via a more formal education <br /> curriculum to meet the needs of the special day classes and <br /> individuals. <br /> Tri-City Health Center East County HIV Early Intervention Services providing access $0 --- <br /> to comprehensive HIV primary care for people living with HIV in <br /> Pleasanton and elsewhere in Eastern Alameda County. <br /> Funding would support a Case Manager who will specifically <br /> serve Pleasanton residents. The HSC did not recommend <br /> funding due to the low number of residents to be served and <br /> service provider's location in south county. <br /> Tri-Valley Haven Assistance and counseling with completion and filing of $11,000 Gen Fund <br /> Temporary Restraining Orders,visitation and custody <br /> arrangements,and court accompaniment. <br /> Tri-Valley Haven Operation of Shiloh emergency shelter and domestic violence $16,000 Gen Fund <br /> services to women and children survivors of family violence, <br /> including crisis line assistance and referral services. <br /> Tri-Valley Haven Operation of Sojourner House homeless shelter providing $30,000 Gen Fund <br /> shelter,homeless services(including counseling,case <br /> management,life skills classes,outreach and referrals). <br /> Page 6 of 11 <br />
