Name of Recipient Agreement No.: XX-XXX-550 Project No.: C-06-XXXX-XXX agency. the amount, type of assistance(grants, loans, etc.), and a description of the facilities, components, and items the funds were used for; list the power and maintenance costs associated with the Project for the period, indicate current plans and programs for use of any Project capacity not under contract, summarize Project financial experience, describe compliance with any special conditions of this Agreement; describe direct and indirect benefits of the project to the state/local water supply and economy; describe other benefits and challenges arising from the project; and provide such other information as may be reasonably required to evaluate Project benefits and use of Project facilities. 2.17 Federal Disadvantaged Business Enterprise(DBE) Reporting. The Recipient agrees to report DBE utilization to the Division on the DBE Utilization Report, State Water Board Form DBE UR334. The Recipient must submit such reports to,tlie Division annually within ten (10) calendar days following October 1 until such time as the"Notice of Completion" is issued. The Recipient agrees to comply with 40 CFR§ 33.301. A 2.18 Records. ' (a) Without limitation of the requirement to maintain Project accounts in.accordance with generally accepted accounting principles, the Recipient agrees to: " \ (1) Establish an official file for the Project which.hall adequately document all'significant actions relative to the Project;`., \ / ' (2) Establish separate accoun s.whidh-wiltadequately and'accurately depict all amounts received and expended on the,Ptojedt including all assistance funds received under this Agreement; , —� 1 ��. ''� A� / . 1 ; (3) Establish/separate accounts which will adequately depict.all income received which is attributable to the Project,'specifically)ncluding any income attributable to assistance funds disbursed under this Agreeent; A �' (4):Es'tablish.an accounting system which will accurately depict final total costs of the Project, including both direct.and indirect costs ti (5).Establish such.accounts`andrmaintain such records as may be necessary for the State to fulfill federal reporting requirements, including any and all reporting requirements under federal tax statutes,or'regulations; and (6) If Force Account is used(by the Recipient for any phase of the Project, other than for planning;design, and construction engineering and administration provided for by allowance>accounts'wili'be established which reasonably document all employee hours charged to the Project and the associated tasks performed by each employee. Indirect Force Account'costs are not eligible for funding. (b) The Recipient shall be required to maintain separate books, records and other material relative to the Project. The Recipient shall also be required to retain such books, records, and other material for itself and for each contractor or subcontractor who performed work on this project for a minimum of thirty-six (36) years after Project Completion. The Recipient shall require that such books, records, and other material be subject at all reasonable times (at a minimum during normal business hours) to inspection, copying, and audit by the State Water Board, the Bureau of State Audits, the United States Environmental Protection Agency (USEPA), the Office of Inspector General, the Internal Revenue Service, the Governor, or any authorized representatives of the aforementioned, and shall allow interviews during normal business hours of any employees who might reasonably have information related to such records.
