Education/Outreach—In Kind <br /> • Develop and maintain a Dental referral services directory for low <br /> income seniors, updated biannually. <br /> • Facilitate six (6) community outreach activities for seniors to educate <br /> them about available dental resources in community. <br /> • Alcohol and Drug Abuse Prevention <br /> • Exercise and Nutrition Classes <br /> Metrics <br /> SSPTV will work with City staff to determine metrics to be established to collect data to evaluate <br /> performance. <br /> Compensation <br /> Monthly payments will be made based upon invoices received. Back up documentation is to be <br /> provided for all expenses.
