EXHIBIT A Scope of Consultant's Services Senior Support Program of the Tri-Valley (SSPTV) Scope of Work 2015-2016 Senior Support Program of the Tri-Valley (SSPTV) shall provide the following services: Friendly Visiting, Case Management, In-home Caregiver Registry, Family Caregiver Support, Alcohol and Drug Abuse Prevention, Education and Outreach, Exercise and Nutrition Classes, Health Screenings, Information and Referral to the Pleasanton Senior residents to be reimbursed up to $88,000 for program expenditures. Specific Services to be performed according to this Agreement for reimbursement are: Case Management Services-$49,000 • Home visit assessments with follow-up care plans including recommendations to support senior independence and viability • On-site, drop-in case management and crisis intervention services available Monday-Friday, 9:00am — 5:00pm (closed from 12:00- 1:00pm) at 5353 Sunol Blvd., Pleasanton, California. This would include having a qualified staff person available to make immediate resolutions and/or referrals Foot Care and Health Screenings—$10,000 • Provide weekly foot care appointments at Pleasanton Senior Center • Work with City Staff to determine expansion of program to accommodate a minimum of 26 residents with a goal of eliminating the waiting list for Pleasanton Residents. • Conduct bimonthly health screenings including, but not limited to: diabetes, blood pressure, hemoccult, and urine. In Home Registry and Family Caregiver Support Services -$11,000 • Provider will visit and assess client's needs, and upon assessment, provide a list of pre-screened workers for in-home supportive services. Will require a minimum of fifteen (15) hours per week. Friendly Visiting- $18,000 • Provide companionship through home visits and telephone calls.
