EXHIBIT A <br /> Scope of Consultant's Services <br /> Senior Support Program of the Tri-Valley (SSPTV) <br /> Scope of Work 2015-2016 <br /> Senior Support Program of the Tri-Valley (SSPTV) shall provide the following services: Friendly <br /> Visiting, Case Management, In-home Caregiver Registry, Family Caregiver Support, Alcohol and Drug <br /> Abuse Prevention, Education and Outreach, Exercise and Nutrition Classes, Health Screenings, <br /> Information and Referral to the Pleasanton Senior residents to be reimbursed up to $88,000 for program <br /> expenditures. <br /> Specific Services to be performed according to this Agreement for reimbursement are: <br /> Case Management Services-$49,000 <br /> • Home visit assessments with follow-up care plans including <br /> recommendations to support senior independence and viability <br /> • On-site, drop-in case management and crisis intervention services <br /> available Monday-Friday, 9:00am — 5:00pm (closed from 12:00- <br /> 1:00pm) at 5353 Sunol Blvd., Pleasanton, California. This would <br /> include having a qualified staff person available to make immediate <br /> resolutions and/or referrals <br /> Foot Care and Health Screenings—$10,000 <br /> • Provide weekly foot care appointments at Pleasanton Senior Center <br /> • Work with City Staff to determine expansion of program to <br /> accommodate a minimum of 26 residents with a goal of eliminating <br /> the waiting list for Pleasanton Residents. <br /> • Conduct bimonthly health screenings including, but not limited to: <br /> diabetes, blood pressure, hemoccult, and urine. <br /> In Home Registry and Family Caregiver Support Services -$11,000 <br /> • Provider will visit and assess client's needs, and upon assessment, provide <br /> a list of pre-screened workers for in-home supportive services. Will <br /> require a minimum of fifteen (15) hours per week. <br /> Friendly Visiting- $18,000 <br /> • Provide companionship through home visits and telephone calls.
