Assumptions: <br /> • The APE survey area will be limited to the areas encompassed by Lions Wayside Park <br /> and Delucchi Park. The field survey will require two cultural resource specialists during a <br /> 10-hour day, including travel tithe. <br /> • Given the nature of the proposed work,it is assumed that a formal significance evaluation <br /> of the former firehouse, bandstand, ditch, and other features will not be required for the <br /> purposes of determining impacts/effects. <br /> • No archaeological test excavations will be conducted. <br /> • Up to one archaeological site will be recorded on DPR 523 forms. <br /> • One draft and one final technical report will be prepared. One set of comments will be <br /> received on the draft report to incorporate into the final report. <br /> Task Cost: $10,215 <br /> Task 5: Prepare California Department of Fish and Wildlife,Fish and Game Code Section <br /> 1602 Streambed Alteration Notification <br /> Pursuant to Fish and Game Code Section 1602, a project applicant proposing to divert or obstruct <br /> the natural channel flow or substantially change the bed, channel, or bank of any river, stream, or <br /> lake designated by CDFW, or use any material from the lakebed, must first notify CDFW. <br /> Notification to CDFW for alteration or modification of CDFW jurisdictional rivers, streams, or <br /> lakes is handled through the preparation and submittal of a Streambed Alteration Agreement <br /> Notification. AECOM will prepare a Streambed Alteration Agreement Notification(form <br /> FG2023) for the proposed project. The notification will include information on the project, <br /> including: <br /> • a complete project description; <br /> • assessor parcel numbers, names, addresses, and contact information for land owners <br /> within the project area; <br /> • at least 30%design drawings for the project(in CADD or ArcGIS); <br /> • quantification of riparian trees and vegetation to be removed as part of the project(see <br /> below); <br /> • information on federal-and state-listed species that could be affected by the project; <br /> • plans showing the project construction area, staging areas, and temporary work spaces; <br /> • project schedule; <br /> • a copy of the CEQA compliance document(a Notice of Determination of the certified <br /> CEQA document is preferred). It is assumed in this scope of work that the CEQA <br /> document that the City certifies will meet this requirement; <br /> • a description of the construction methods used to install project elements; and <br /> • proof of fee payment. <br /> 11 ' Page <br />
