The City recently redesigned its grant programs into a single consolidated grant program called the <br /> Housing and Human Services Grant(HHSG).The HHSG program was developed in 2009 as a refinement <br /> and consolidation of four separate grant programs: <br /> 1. Federal Community Development Block Grant(CDBG) Funds <br /> 2. Federal HOME Investment Partnership Program (HOME) Funds <br /> 3. City Lower Income Housing Funds for Housing <br /> 4. City General Funds for Human Services(formerly"City Grant") <br /> The consolidation of these four funding sources into a single streamlined grant program has created <br /> greater efficiencies in the allocation of limited funding sources as well as the coordination and delivery <br /> of human services.While each source has specific priorities and requirements,there is an overarching <br /> priority for funding programs and services that benefit low income residents and for addressing gaps in <br /> housing and services. <br /> The City of Pleasanton has continued to take a lead role in the activities of the Tri-Valley Affordable <br /> Housing Committee (TVAHC),which is made up of representatives of cities in the Livermore Amador <br /> Valley area (Pleasanton, Livermore, Dublin,San Ramon, and Danville).The TVAHC has actively <br /> participated in annual outreach activities promoting public awareness of affordable housing in the Tri- <br /> Valley area,working with the East Bay Housing Organizations (EBHO). In addition,the-VAHC initiated <br /> the successful establishment of the Tri-Valley Housing Opportunity Center(TVHOC)which opened in <br /> 2005.The TVHOC serves as a "clearing house," initially for ownership programs and ultimately for a <br /> continuum of housing assistance of different types in the Tri-Valley. <br /> Describe coordination with the Continuum(s)of Care that serves the jurisdiction's area in <br /> determining how to allocate ESG funds,develop performance standards for and evaluate <br /> outcomes of projects and activities assisted by ESG funds, and develop funding, policies and <br /> procedures for the operation and administration of HMIS <br /> [by Alameda County] <br /> Describe consultation with the Continuum(s)of Care that serves the State in determining how <br /> to allocate ESG funds,develop performance standards for and evaluate outcomes of projects <br /> and activities assisted by ESG funds,and develop funding, policies and procedures for the <br /> operation and administration of HMIS <br /> [by Alameda County] <br /> 2. Agencies,groups, organizations and others who participated in the process and <br /> consultations <br /> Annual Action Plan 7 <br /> 2013 <br /> OMB control No:2506-0117(exp.07/31/2015) <br />
