Invitation to Bid on "As Needed Services" • Contractor will use 100% imported backfill as per City Specifications. • Contractor will off haul and dispose of any unsuitable material to include dump fees. • Contractor will be responsible to obtain encroachment permits, if necessary, and a schedule of inspection, Underground Service Alerts, and a current Cal-OSHA excavation permit (The City of Pleasanton will notify USA for emergency work only). • Contractor must submit a traffic control plan to the City of Pleasanton Operations Services Department, Utility Division Lead Operator, for approval 48 hours prior to scheduled work. • Contract crews shall be identified at all times either by a Company uniform, or by a safety vest worn outside other clothing with the Company name on back. Vehicles will have a Company sign on both front doors SCOPE OF SERVICES The following outline provides a scope of work for "as needed services" in the following areas in addition to the general contract provisions above. You are invited to bid on any section of services that your firm is qualified and equipped to perform for the City. City Specifications can be obtained at the Department of Community Development/Engineering for the City of Pleasanton, 200 Old Bernal, Pleasanton, CA at a cost of$40 per set. BUILDING MAINTENANCE -Facility Repairs • Repair facilities on an "as needed" basis by the direction of Support Services personnel and also on a 24-hour emergency basis including overtime if required. • Provide the City with all pesticide related documentation, as to chemicals used, date of service and location. Any and all MSDS as required for products used. UTILITIES- Sewer System Repairs • Contractor will repair and/or replace sewer mains and or structures "as required" on a scheduled and 24-hour emergency basis including overtime if required according to prescribed specifications and City of Pleasanton Standard Details. • Contractor must be qualified and furnish a statement of qualifications and experience to provide sewer main and structure repair services. • Contractor shall diligently perform the services and furnish the personnel, materials, equipment, vehicles, safety devices, traffic control equipment and plans for sewer main and structure repairs and replacement work. • Contractor, after being advised by the Utilities Division on a work area, must notify residents or businesses in writing a notice to perform services for the City a minimum of 48 hours prior to work being started. Contractor must also provide a copy of the notice to the Utilities Division. 3
