PUBLIC HEARINGS <br /> 4. Review of Community Development Block Grant (CDBG) Consolidated Annual <br /> Performance and Evaluation Report (CAPER) for Fiscal Year 2010 <br /> Mr. Erickson reviewed the CAPER process and the requirement to submit an annual report to <br /> HUD regarding the CDBG program requirements and the collection of information from each of <br /> the agencies that received grants. <br /> Chairperson Nagler opened the meeting for agency presentations at 6:40 p.m. <br /> Abode Services/TVHSP Case Management Services — Penny Andersen — Ms Andersen <br /> thanked the Commission for the funds received that provided case management and housing <br /> assistance to Pleasanton residents. A total of five (5) households received rental assistance, <br /> two (2) participants completed their educational goals and were able to find work, and one (1) <br /> obtained a full scholarship to the University of Kentucky. One (1) vacancy currently exists in the <br /> program. <br /> Axis Community Health/Medical Visits for Low-Income Residents — Sue Compton — Ms. <br /> Compton thanked the Commission for funding received. She advised that 165 medical visits <br /> were provided for 156 unduplicated low-income uninsured Pleasanton residents. <br /> Axis Community Health/Automated Patient Messaging Systems — Sue Compton — The <br /> Teleminder systems was installed and has helped reduce the 'no-show" rate by 10% and helped <br /> increase the agency's ability to provide medical by filling more appointment slots. The system <br /> has also helped in reducing the amount of staff time spent calling patients to remind them of <br /> appointments. <br /> Axis Community Health/Health Care Access for Uninsured — Sue Compton — A total of 522 low- <br /> income Pleasanton residents were enrolled in health plans that provided them access to medical <br /> care. Ms. Compton provided an update on the renovation completion of the agency's Behavioral <br /> Health Building that has increased the capacity for storing patient records. She encouraged <br /> members of the Commission to visit this improved building at Axis. <br /> Bay Area Community Services/Creative Livings Services — Kirsten Smith — Advised that funds <br /> received enabled BACS to provide services to 68 adults with psychiatric disabilities. BACS also <br /> provides a public service that benefits 100% extremely low-income clients. A major <br /> accomplishment of the agency involves activities that are designed to create and retain jobs for <br /> clients. She advised the Commission that the Creative Living Center (CLC) was transitioning to <br /> a wellness center that helps older adults get back into the community through housing and <br /> employment services. <br /> Child Care Links/Community Services Project — Nicole Roman — Thanked the Commission for <br /> funding this project which helps Child Care Links bring awareness into the community about <br /> child care services.
