b. Date of birth; <br /> c. California driver's license number; <br /> d. Copy of valid medical certificate (if required by classification); <br /> e. Job description and/or title; <br /> f. Current home address and telephone number; and <br /> g. Types of trucks the driver has been trained and is authorized to operate. <br /> E. The PPD may inspect all Operator records without notice during normal business hours. <br /> F. An Operator shall permit the designee of the PPD to make copies of business records at <br /> its place of business or remove business records for the purpose of reproduction. The <br /> PPD shall provide a receipt for all original records removed from the place of business. <br /> G. An Operator shall maintain and make available for inspection to PPD records for a <br /> period of two (2)years plus the current term of the TSA. <br /> H. Failure of an Operator to comply with the inspection requirements shall be cause for <br /> disciplinary action,which shall include, at a minimum, suspension. <br /> 7. FINANCIAL INTEREST <br /> A. No Operator or applicant shall be directly or financially involved in the towing related <br /> business of any other Operator or applicant conducting business within the city limits of <br /> the City of Pleasanton. <br /> B. The sale or transfer of the controlling interest in a tow service shall immediately <br /> terminate the TSA. A new owner may apply for the Rotation Tow Program at any time <br /> during the remainder of the current TSA term, regardless of the enrollment period. <br /> C. A suspended Operator shall not be eligible for a rotation listing for the duration of the <br /> suspension. A terminated Operator shall not be eligible for a rotation listing for the <br /> duration of the TSA. This provision applies to the Operator working in any capacity <br /> within any tow business or operating any tow business, and to the tow service, even if <br /> operated under new ownership. <br /> D. No Operator or applicant shall share with or borrow equipment from any other <br /> Operator involved with the Rotation Tow Program (excluding equipment which may be <br /> unavailable due to repairs, equipment replacements). <br /> 2011-2013 TSA Page 9 of 30 <br />
