CONTRACT FOR SERVICES <br /> THIS AGREEMENT is made and entered into this 1s`day of July 2011, by and between <br /> the CITY OF PLEASANTON, a municipal corporation (hereinafter referred to as the "City"), <br /> and Innovative Claim Solutions, Inc. whose address is P.O. Box 5128, San Ramon, CA 94583, <br /> and telephone number is 925-327-8050, (hereinafter referred to as the "Consultant"). <br /> WITNESSETH: <br /> A. Consultant is qualified to provide, and experienced in providing claims <br /> management and administration of workers' compensation self-insurance plans, and has offered <br /> its services for the purposes specified in this Agreement. <br /> B. In the judgment of the City, it is necessary and advisable to employ the services of <br /> Consultant for the purposes provided herein. <br /> NOW THEREFORE, in consideration of the mutual covenants, agreements and <br /> conditions contained herein, City and Consultant agree as follows: <br /> 1. Consultant's Services. Consultant shall diligently perform the services and <br /> furnish the materials, reports, drawings, and related materials as hereinafter set forth: <br /> a. Consultant shall perform all work described in Exhibit "A", Scope of <br /> Work, attached hereto and incorporated herein by this reference. <br /> b. Consultant shall meet with City staff as necessary to complete the scope of <br /> work outlined in Section 1 (a) above. <br /> 2. City Services. In order to assist Consultant in this work, City shall provide, if <br /> necessary, support and items mentioned in Exhibit"A". <br /> 3. Staff. Consultant shall assign adequate staff to maintain and adjudicate claims in <br /> a timely and efficient manner. The Human Resources Department will be the primary point of <br /> contact for the City of Pleasanton. <br /> 4. Sufficiency of Consultant's Work. All reports prepared for City shall be <br /> adequate and sufficient to meet the purposes for which they are prepared. <br /> 5. Ownership of Work. All reports, work data, and all other documents <br /> completed or partially completed by Consultant in the performance of this Agreement shall <br /> become the property of City. <br /> 6. Terms. Consultant shall begin work upon the direction of the Human Resources <br /> Department, City of Pleasanton. The work as described in Exhibit "A", Scope of Work, shall be <br />
