Exhibit B Conditions of Approval Bridge Housing Corporation 100 Junipero Street Case PUD-20 Development shall be substantially as shown on the development plans, Exhibit "A", dated "Received July 15, 2002" on file with the Planning Department, except as modified by the following conditions. Minor changes to the plans and conditions may be allowed subject to the approval of the Planning Director if found to be in substantial conformance to the approved exhibits. The permitted use of the site shall be an assisted living facility only. Any proposed change to the land use shall require prior approval of a PUD modification. The maximum occupancy of this facility shall be 125 residents. Any proposed changes in the operation of this assisted living facility shall be submitted for review and approval by the Planrdng Director. The Planning Director may approve the modification or refer the item to the Planning Commission and/or the City Council if judged to be substantial. The number of affordable units provided shall be consistent xvith the applicant's lease agreement with the City. which shall be executed by all parties prior to issuance of a building permit for the project. Prior to issuance of a building permit, the name of the project shall be submitted to the Planning Director for review and approval. The name shall not be similar to an existing assisted living or senior housing complex in the City. The operator of the facility shall maintain at all times a handicapped- accessible van sufficient to ensure that the use of the Pleasanton Paratransit Service wilt not be necessary for all of the residents' transit needs (e.g., medical, shopping, personal services, etc.). If necessary to adequately serve the residents, the operator shall add additional vehicles. The operator of this facility shall keep in full force and effect all required State, County, and agency licenses required to operate this facility.
