Exhibit B <br />Conditions of Approval <br />Bridge Housing Corporation <br />100 Junipero Street <br /> Case PUD-20 <br /> <br />Development shall be substantially as shown on the development plans, <br />Exhibit "A", dated "Received July 15, 2002" on file with the Planning <br />Department, except as modified by the following conditions. Minor <br />changes to the plans and conditions may be allowed subject to the approval <br />of the Planning Director if found to be in substantial conformance to the <br />approved exhibits. <br /> <br />The permitted use of the site shall be an assisted living facility only. Any <br />proposed change to the land use shall require prior approval of a PUD <br />modification. <br /> <br />The maximum occupancy of this facility shall be 125 residents. Any <br />proposed changes in the operation of this assisted living facility shall be <br />submitted for review and approval by the Planrdng Director. The Planning <br />Director may approve the modification or refer the item to the Planning <br />Commission and/or the City Council if judged to be substantial. <br /> <br />The number of affordable units provided shall be consistent xvith the <br />applicant's lease agreement with the City. which shall be executed by all <br />parties prior to issuance of a building permit for the project. <br /> <br />Prior to issuance of a building permit, the name of the project shall be <br />submitted to the Planning Director for review and approval. The name <br />shall not be similar to an existing assisted living or senior housing complex <br />in the City. <br /> <br />The operator of the facility shall maintain at all times a handicapped- <br />accessible van sufficient to ensure that the use of the Pleasanton Paratransit <br />Service wilt not be necessary for all of the residents' transit needs (e.g., <br />medical, shopping, personal services, etc.). If necessary to adequately serve <br />the residents, the operator shall add additional vehicles. <br /> <br />The operator of this facility shall keep in full force and effect all required <br />State, County, and agency licenses required to operate this facility. <br /> <br /> <br />
