MEETING PHASE: <br /> 12. Prior to the Human Services Commission meeting, did you receive a <br /> copy of the agenda and staff report and, if applicable, did you have <br /> sufficient opportunity to discuss the report with staff prior to the <br /> meeting? <br /> • Timely receipt of Agenda; no communication with staff prior <br /> • We received the agenda but don't believe we received the staff report. As <br /> a result we did not discuss the report with the staff prior to the meeting <br /> 13.Do you have any comments regarding the format, organization and <br /> conduct of the Human Services Commission meeting? <br /> • No Comment <br /> • Since our offices are located in the Hayward area, it seemed odd to attend <br /> the meeting for questions when the Commission did not have any <br /> questions for our program. Would be nice to know if it was necessary to <br /> attend or if there were not questions for us <br /> • The organization is excellent; you've streamlined the meeting and I think it <br /> runs very smoothly <br /> • All agencies, new and returning, should make a presentation in order to <br /> not give new applicant agencies an unfair advantage <br /> • Lost time attending <br /> 14. Do you feel that you had an adequate opportunity to present information <br /> regarding your application (either via a scheduled presentation or <br /> during the public comments section)? <br /> • 17 surveys answered Yes <br /> • 2 surveys had no comment <br /> • No, I think all agencies, new and returning, should make a presentation in <br /> order to not give new applicant agencies an unfair advantage <br /> • Would be nice to allow a minute or so to briefly summarize our <br /> achievements <br /> GENERAL COMMENTS /RECOMMENDATIONS: <br /> 15. Please take an opportunity to provide us with any general or specific <br /> feedback regarding the HHSG application process. <br /> • Really like ZoomGrants; loved the fact I didn't have to carry 12 copies to <br /> City Hall. Debbie and Scott seemed very well - prepared at the workshop <br /> and all questions were answered completely. Feel I can contact staff with <br /> Page 31 of 32 <br />
