Resolution No. PC-2002-51 <br />Page 5 <br />Exhibit B <br />Conditions of Approval <br />Bridge Housing Corporation <br />100 Junipero Street <br />Case PUD-20 <br />August 14, 2000 <br />Development shall be substantially as shown on the development plans, Exhibit <br />"A", dated "Received July 15, 2002" on file with the Planning Department, except <br />as modified by the following conditions. Minor changes to the plans and <br />conditions may be allowed subject to the approval of the Planning Director if <br />found to be in substantial conformance to the approved exhibits. <br />2. The permitted use of the site shall be an assisted living facility only. Any <br />proposed change to the land use shall require prior approval of a PUD <br />modification. <br />3. The maximum occupancy of this facility shall be 125 residents. Any proposed <br />changes in the operation of this assisted living facility shall be submitted for <br />review and approval by the Planning Director. The Planning Director may <br />approve the modification or refer the item to the Planning Commission and/or the <br />City Council if judged to be substantial. <br />4. The number of affordable units provided shall be consistent with the applicant's <br />lease agreement with the City, which shall be executed by all parties prior to <br />issuance of a building permit for the project. <br />5. Prior to issuance of a building permit, the name of the project shall be submitted to <br />the Planning Director for review and approval. The name shall not be similar to an <br />existing assisted living or senior housing complex in the City. <br />6. The operator of the facility shall maintain at all times ahandicapped-accessible <br />van sufficient to ensure that the use of the Pleasanton Paratransit Service will not <br />be necessary for all of the residents' transit needs (e.g., medical, shopping, <br />personal services, etc.). If necessary to adequately serve the residents, the operator <br />shall add additional vehicles. <br />
