Resolution No. PC-2002-49 <br />Page 4 <br />EXHIBIT "B" <br />CONDITIONS OF APPROVAL <br />"CENTER FOR SPEECH, LANGUAGE, OCCUPATIONAL THERAPY" <br />PCUP-57 <br />6155 STONERIDGE DRIVE <br />The proposed use covered by this approval shall be in substantial conformance to <br />the plans and narrative, shown as Exhibit A, dated "Received July 22, 2002", on <br />file with the Planning Department, except as modified by the following <br />conditions. Minor alterations may be approved by the Planning Director if found <br />to be in covered by substantial conformance with this approval. <br />2. All conditions of Cases PUD-80-16 and PUD-80-16-18D shall remain in full <br />force and effect. <br />3. The business may operate during the following hours: <br />Monday through Friday: from 8:00 a.m. to 6:00 p.m. <br />Saturday: By Appointment only <br />In the event that the applicants propose to modify the hours of operation or <br />other aspects of the business operation, or modify the exterior building or <br />site, the modification shall be subject to the review of the Planning <br />Director. The Planning Director may approve the modification or refer the <br />matter to the Planning Commission if judged to be substantial. <br />4. At no time there shall be more than four individual consultations held <br />concurrently with group sessions. If, in the future, adult group sessions are <br />to be held at this location, representatives from CSLOT shall contact the <br />Planning Department prior to commencement. At the discretion of the <br />Planning Director, an additional assessment of parking needs may be <br />required at that time. <br />CSLOT shall have a 30-minute break between group sessions so that <br />pazents/guardians can have adequate time to pickup and drop-offtheir <br />children without creating a parking or circulation problem in the parking <br />lot. <br />6. Prior to occupancy, the applicant shall post sign, west of the building <br />entrance, directly outside the CSLOT's leased suite, indicating that the two <br />pakking spaces are designated as a 10-minute drop-off/pick-up zone for <br />CSLOT parents/guardians. The two parking spaces shall be striped, and <br />wheel stops shall be installed. <br />
