EXHIBIT B <br /> <br /> CITY COUNCIL <br /> <br /> Conditions of Approval <br /> PUD-90-7 <br /> November 20, 1990 <br /> <br />1. The development plan shall substantially conform to the plans <br /> and elevations dated October 10, 1990 on file with the <br /> Planning Department, except as modified herein. <br /> <br />2. This PUD approval is limited to the senior care facility use <br /> as shown on the approved development plan and as described in <br /> the applicantts letters. dated February 7, and June 8, 1990. <br /> The applicant shall maintain the types of services and care as <br /> stated in these letters, plus the following conditions: <br /> <br /> a. The maximum occupancy for the senior care facility shall <br /> be 70 residents. <br /> <br /> b. The applicant shall keep in full force and effect all <br /> required state and county licenses. <br /> <br /> c. The applicant shall maintain at all times a van pool <br /> service sufficient to ensure that use of the City's Dial- <br /> a-Ride service will not be necessary for residents of <br /> this facility. If necessary to adequately serve the <br /> residents, the applicant shall add a second van for the <br /> facility. <br /> <br /> Any substantial deviation from the level of care and service <br /> described in the above-referenced letters or conditions shall, <br /> at the discretion of the Planning Director, be referred to the <br /> planning Commission for hearing to consider the possible <br /> revocation of the use portion of this PUD approval. <br /> <br />3. The applicant shall submit a detailed landscape plan for the <br /> project for the review and approval of the Design Review Board <br /> prior to the issuance of a building permit. The landscaping <br /> palette and design shall resemble that of the landscape plan <br /> approved by the Design Review Board on June 20, 1990. The <br /> plan shall indicate the specific planting locations of the <br /> shrubbery, which shall be planted between 2 and 5 feet apart <br /> on. center in 5 gallon size containers. <br /> <br /> <br />
