EXHIBIT B CITY COUNCIL Conditions of Approval PUD-90-7 November 20, 1990 1. The development plan shall substantially conform to the plans and elevations dated October 10, 1990 on file with the Planning Department, except as modified herein. 2. This PUD approval is limited to the senior care facility use as shown on the approved development plan and as described in the applicantts letters. dated February 7, and June 8, 1990. The applicant shall maintain the types of services and care as stated in these letters, plus the following conditions: a. The maximum occupancy for the senior care facility shall be 70 residents. b. The applicant shall keep in full force and effect all required state and county licenses. c. The applicant shall maintain at all times a van pool service sufficient to ensure that use of the City's Dial- a-Ride service will not be necessary for residents of this facility. If necessary to adequately serve the residents, the applicant shall add a second van for the facility. Any substantial deviation from the level of care and service described in the above-referenced letters or conditions shall, at the discretion of the Planning Director, be referred to the planning Commission for hearing to consider the possible revocation of the use portion of this PUD approval. 3. The applicant shall submit a detailed landscape plan for the project for the review and approval of the Design Review Board prior to the issuance of a building permit. The landscaping palette and design shall resemble that of the landscape plan approved by the Design Review Board on June 20, 1990. The plan shall indicate the specific planting locations of the shrubbery, which shall be planted between 2 and 5 feet apart on. center in 5 gallon size containers.
