Ordinance No. 1584 <br />Page Eight <br /> <br /> A. The Driver's name, home address, date of birth, and phone <br /> number; a copy of the appropriate class driver's license; <br /> if the Driver is not a citizen of the United States, a <br /> copy of the Driver's work authorization permit; any <br /> information relative to the Driver's driving record, and, <br /> if any, criminal history; <br /> <br /> B. A statement from the Driver as to whether the Driver is <br /> being hired as an independent contractor or as an <br /> employee of the Taxicab company. If the Driver is hired <br /> as an independent contractor, the Driver must obtain a <br /> City business license; <br /> <br /> C. The Taxicab company the Driver is planning to work for, <br /> and the name, the business address, and business phone <br /> number of the Owner; <br /> <br /> D. The experience of the Driver in the transportation of <br /> passengers; <br /> <br /> E. The educational background of the Driver; <br /> <br /> F. A concise history of the Driver's employment; <br /> <br /> G. Two recent photographs (size one and one-half inches by <br /> one and one-half inches) of the Driver; and <br /> H. An application fee as established by the City Council. <br /> 6.40.150. D~iver's Permit - Denial/Revocation. <br /> The Police Chief shall deny a Driver's permit and shall revoke <br /> a permit upon a finding: <br /> <br /> A. that the Driver is under twenty-one years of age; <br /> <br /> B. that the Driver does not possess a valid Class C license <br /> issued by the State of California; <br /> <br /> C. that the Driver has been convicted of reckless driving or <br /> driving while under the influence of intoxicating <br /> beverages or drug~ <br /> <br /> D. that the Driver has been convicted of a felony or crime <br /> involving moral turpitude; <br /> <br /> E. that the Driver failed to satisfactorily pass the <br /> Driver's written test administered by the police <br /> department; or <br /> <br /> <br />
