Ordinance No. 1584 Page Four J. An application fee as set forth in the Master Fee Resolution No. 92-100, as said Resolution may be amended from time to time; K. A statement of the location where the Taxicabs will be operated from, and what zoning exists for that address or if a zoning change will be required; L. A copy of the certificate of insurance for the business and for the Taxicabs, or a written statement from an insurance company agreeing to provide insurance should the application be granted. This insurance company shall be headquartered in the United States, authorized by the State of California to provide insurance in this state and approved by the City Attorney as to the financial status; M. A copy of the applicant's Federal Communication Commission license to operate a base and mobile radio units; N. A copy of the applicant's bonding policy for its Drivers; O. A copy of the applicant's training program for its Drivers, whether such Drivers are considered independent operators or not; P. A statement of the applicant's personal and business history regarding Taxicab operations/ownershiP; Q. A statement as to whether the applicant has ever been denied a municipal taxi permit, or if such permit was issued, whether it was ever suspended or revoked, and the reasons why; and R. Such other additional information as the City Council or City Manager may require. 6.40.030. Police Investiqation and Report Required. No Certificate shall be granted, nor application for said certificate acted upon, until the Police Chief causes an investigation to be made and the report and findings transmitted to the City Council, to be reviewed concurrent with the application.
