Section 14. Health and Welfare Coverage <br /> <br />14.1 <br /> <br />Hospital and Medical <br /> <br />Effective April 1, 1979, the City shall contribute an <br />amount necessary to provide hospital and medical care <br />benefits under the present Blue Cross Plan or Kaiser S <br />Plan for the individual employee and a portion of the <br />amount necessary to provide coverage for eligible dependents; <br />provided, however, that in no event shall the City be <br />obligated to contribute an amount in excess of Eighty-two <br />Dollars ($82.00) per month per employee to provide the <br />aforesaid coverage. <br /> <br />14.2 Dental <br /> <br />Effective April 1, 1979, the City shall contribute an <br />amount necessary to provide dental care benefits under the <br />City's present Bankers Life Nebraska Plan for the indi- <br />vidual employee and eligible dependents; provided, however, <br />that in no event shall the City be obligated to contribute <br />an amount in excess of Twenty-five Dollars ($25.00) per <br />month per employee to provide the aforesaid coverage. <br /> <br />14.3 <br /> <br />Life, Accidental Death & Dismemberment and Long Term <br />Disability <br /> <br />The City shall contribute an amount necessary to provide <br />life, accidental death and dismemberment, and long term <br />disability insurance under the City's present Standard <br />Insurance Company Group Policy for the individual employee; <br />provided, however, that in no event shall the City be <br />obligated to contribute an amount in excess of Ninety-five <br />Cents (95¢) per One Hundred Dollars ($100) Of monthly <br />salary of the first $2250 of monthly salary not to exceed <br />a total contribution of Twenty-one Dollars and Thirty- <br />seven Cents ($21.37) per month per employee. <br /> <br />Section 15. Retirement <br /> <br />The City shall amend its contract with PERS for the <br />purpose Of providing On January 1, 1981, the "2% at 50" <br />retirement plan to include the City's present 1959 <br />Survivor's benefit option. <br /> <br />Section 16. Uniforms <br /> <br />The City shall provide at its expense required protective <br />clothing and footwear. For new employees the City shall <br />pay fifty percent (50%) of the cost of original uniform <br />requirements: three (3) shirts, three (3) pants, belt, <br />boots, and any other required uniform. <br /> <br />Uniform changes initiated by the City shall be made <br />at the expense of the City. <br /> <br />All employees covered by this agreement shall wear the <br />same uniforms. <br /> <br />Effective April 1, 1979, those employees required to wear <br />a prescribed uniform shall receive an annual uniform <br />maintenance allowance of $190.00 payable on April 1 <br />of each year; provided, however, that if the payment date <br />follows by less than six (6) months the date of appoint- <br />ment of such an employee, no such uniform maintenance <br />allowance for said employee shall be forthcoming until <br />the succeeding annual payment date. <br /> <br />~ll- <br /> <br /> <br />
