19.6 <br /> <br />Doc~_tmentation of Educational Achievement <br /> <br />Evidence of successful completion of a degree or <br />certificate shall consist of either of the following: <br /> <br />Diploma or certificate indicating the major <br />field of study. <br /> <br />Letter from the college registrar certifying <br />award of the degree or certificate and the <br />major field of study. <br /> <br /> Evidence of completion of the required accredited <br /> semester units or quarter hour equivalents may <br /> consist of any of the following: <br /> <br />'1. Cdllege transcript-of grades. .~ <br /> <br /> 2. .Official grade slips from each of the courses. <br /> <br /> 3. Letter from the college registrar certifying <br /> the completion ofeach, c~urse with a grade of <br /> · "C" or better.: <br /> <br />19.7 <br /> <br />Effectiv~ Date of Educational Incentive Pay <br /> <br />This Educ~%ional Incentive Pay Plan shall be effective <br />July I, 1977. <br /> <br />19.8 <br /> <br />Tuition ReimburSement <br /> <br />O~ly employees'enrolled in the Educational Incentive <br />Plan shall be eligible for tuition reimbursement. <br /> <br />Tuition shall be reimbursed in' the amount and at - <br />the rate for-internal degrees levied by Ha!~;ard <br />State University at the time of the employee's <br />enrollment in the courses"'~or which tuition reim- <br />bursement is requested. However, the maximLL!n_amount <br />of tuition reimbursement shall not exceed $300.00 <br />par year per employee. <br /> <br />TuifiSn"rei'mbdf~e~ht'S~all not be receiv~ for a~y <br />indivudual courses (or class) not successfully <br />completed with a grade of "C" or be~ter. <br /> <br />Employees submitting requests for tuition reimburse- <br />ment shall provide'the Personnel Officer an official <br />receipt from the college or university verifying the <br />payment pf tuition and an official transcript of <br />the grades received for which tuition is requested. <br /> <br />19.9 <br /> <br />Book Reimbursement. <br /> <br />Employees enrolled in the Educational'Incentive Plan <br />shall'be eligible for the reimbursement of the books <br />required for those classes taken as part of their <br />approved educational program. The amount reimbursed <br />shall be the purchase price of the books less the monies <br />received from their resale. No reimbursement for books <br />shall be received for any class not successfully completed <br />with a grade of "C" or better. <br /> <br /> <br />
