<br />I EXHIBIT D <br /> <br />SUMMARY OF COLLISIONS ON VINEYARD AT THE ROUNDABOUTS <br /> <br />Collision #1 <br />Driver swerved off roadway to avoid car traveling in opposite direction that crossed the <br />double yellow centerline. The wet pavement prevented the ability to counter steer and <br />the vehicle drove into adjacent ditch where it came to rest after hitting a street sign. <br /> <br />Collision #2 <br />Motorcyclist unfamiliar with roadway hit the entry island. The rider was the lead in a <br />group of 3 riders. The first and second riders hit the entry island. The third was able to <br />come to a stop and help the other two motorcyclists. The actual speed ofthe motorcycles <br />is not clear. The accident report stated a minimum of 34 mph based on the skid, but that <br />the speed could have been significantly greater if the rider used his front and rear break to <br />reduce speed. The rider of this motorcycle was transported to the hospital and does not <br />recall the crash. <br /> <br />Collisiou #3 <br />Motorcyclist unfamiliar with roadway hit the entry island. This is the second motorcycle <br />in the group ofthree. The rider was not injured in the crash. The rider witnessed the <br />hard braking of the lead rider and began to slow, but the rider was unable to avoid the <br />same island that the motorcycle in front of him crashed into. <br /> <br />Collision #4 <br />The driver in this crash slowed to 25 mph to travel around the roundabout when a deer <br />ran across the roadway. The driver swerved to miss the deer and hit the entry island, lost <br />control and came to rest in the center island. <br /> <br />Collision #5 <br />This driver was familiar with the area and was approaching the roundabout and applied <br />the brakes to slow down. The driver lost control due to the wet pavement conditions and <br />impacted the entry island and came to rest in the central island. <br /> <br />Collisiou #6 <br />Driver estimated his speed at around 55 mph. Posted limit is 40 miles per hour. The <br />driver did not hit the entry island but was going too fast to be able to negotiate <br />roundabout and drove straight through the center island where the driver lost control and <br />came to rest approximately 100 feet into the dirt field past the roundabout. <br /> <br />Collision #7 <br />DUI - Driver avoided entry island, but struck center island coming to rest beyond the <br />intersection in the field. <br /> <br />Collision #8 <br />Truck driver intending to make a delivery to Nevada Street was looking for a place to <br />turn around on Vineyard Avenue. The driver attempted to make a U-turn around the <br />roundabout but was unsure ifhis trailer would be able to complete the turn behind him. <br />
