and subcontract, or other costs. Substantiating documentation such as payroll records, receipts, invoices, or the like, must be submitted in support of each claimed cost. Any Claim for an extension of time or delay costs must be substantiated with schedule analysis and narrative depicting and explaining claimed time impacts. (B) Claim Format. A Claim must be submitted in the following format: (1) General introduction, specifically identifying the submission as a "Claim" submitted under this Section 7. (2) Relevant background information, including identification of the specific demand at issue, and the date of City's rejection of that demand. (3) Detailed explanation of the issue(s) in dispute. For multiple issues, separately number and identify each issue and include the following for each separate issue: (a) The background of the issue, including references to relevant provisions of the Contract Documents; (b) A succinct statement of the matter in dispute, including Contractor's position and the basis for that position; (c) A chronology of relevant events; (d) The identification and attachment of all supporting documents (see subsection (A), above, on Substantiation); and (e) Use of a separate page for each issue. (4) Summary of issues and damages. (5) The following certification, executed by the Contractor's authorized representative: "The undersigned Contractor certifies under penalty of perjury that its statements and representations in this Claim are true and correct. Contractor warrants that this Claim is comprehensive and complete as to the matters in dispute, and agrees that any costs, expenses, or delay claim not included herein are deemed waived. Contractor understands that submission of a Claim which has no basis in fact or which Contractor knows to be false may violate the False Claims Act (Government Code Section 12650 et seq.)."
