4-06. Workers' Compensation: <br />(a) In accordance with the provisions of Section 1860 of the California <br />Labor Code, the Contractor's attention is directed to the requirement <br />that in accordance with the provisions of Section 3700 of the <br />California Labor Code, every contractor will be required to secure the <br />payment of compensation of his or her employees. <br />(b) In accordance with the provisions of Section 1861 of the California <br />Labor Code, each Contractor to whom a public works contract is <br />awarded shall sign and file with the awarding body the following <br />certification prior to performing the work of the contract: "I am aware <br />of the provisions of Section 3700 of the Labor Code which require <br />every employer to be insured against liability for worker's <br />compensation or to undertake self-insurance in accordance with the <br />provisions of that code, and I will comply with such provisions before <br />commencing the performance of the work of this contract." <br />4-07. Prime Contractor Job Site Postings. Pursuant to Labor Code Section 1771.4, <br />Contractor is required to post all job site notices prescribed by law or regulation. The <br />contractor shall comply with all applicable provisions of section 16451 (d) of California <br />Labor Code relating to the posting of job site notices prescribed by regulation. <br />4-08. Insurance Requirements for Contractors: BIDDER'S ATTENTION IS <br />DIRECTED TO THE INSURANCE REQUIREMENTS BELOW. IT IS HIGHLY <br />RECOMMENDED THAT BIDDERS CONFER WITH THEIR RESPECTIVE <br />INSURANCE CARRIERS OR BROKERS TO DETERMINE IN ADVANCE OF BID <br />SUBMISSION THE AVAILABILITY OF INSURANCE CERTIFICATES AND <br />ENDORSEMENTS AS PRESCRIBED AND PROVIDED HEREIN. IF AN <br />APPARENT LOW BIDDER FAILS TO COMPLY STRICTLY WITH THE <br />INSURANCE REQUIREMENTS, THAT BIDDER MAY BE DISQUALIFIED FROM <br />AWARD OF THE CONTRACT. <br />Contractor shall procure and maintain for the duration of this contract, including one year <br />maintenance period, contract insurance against claims for injuries to persons or damages <br />to property which may arise from or in connection with the performance of the work <br />hereunder by the Contractor, the Contractor's agents, representatives, employees or <br />subcontractors. The cost of such insurance shall be included in the Contractor's bid. <br />(a) Minimum Scope of Insurance <br />Coverage shall be at least as broad as: <br />1. Insurance Services Office form number CG 00 01 (ED. 1/96) covering <br />Commercial General Liability and name the City as additional insured. <br />2. Insurance Services Office form number CA 00 01 (Ed. 12/93) covering <br />Automobile Liability, Code I "any auto." <br />Valley Avenue Soundwall Repair Project (REBID), CIP No. 18437 <br />Page 38 <br />
