Utilization of compensatory time off shall be by mutual <br />agreement between the police chief and the employee. Once <br />per year, employees may be paid for all accrued compensatory <br />time. Said payments shall be made on the first.pay day in <br />December. Written requests for payment must be made by the <br />employee fifteen days in advance of the first pay day in December. <br /> <br />Section 8. Holidays <br /> <br /> Employees working shift work shall be compensated each <br />year for working holidays through receipt of holiday in-lieu <br />pay. Effective October 1, 1978, holiday in-lieu pay shall be <br />seven and five-tenths percent (7.5%) of the employee's annual <br />salary exclusive of any overtime and shall be in addition to <br />the employee's regular monthly salary. Holiday in-lieu pay <br />shall be paid in two equal installments, the first installment <br />paid on the first pay day after November 1, and the second <br />installment paid on the first pay day after May 1. <br /> <br />Employees shall receive pro-rated pay upon termination. <br /> <br />Section 12. Health and Welfare Coverage <br /> <br />12.1 Hospital and Medical <br /> <br /> Effective May 1, 1985, the City shall contribute an <br />amount necessary to provide hospital and medical care <br />benefits under either the present Blue Cross Plan, Kaiser S-1 <br />Plan or Foundation Health Plan Plan I for the individual <br />employee and a portion of the amount necessary to provide <br />coverage for eligible dependents; provided, however, that in no <br />event shall the City be obligated to contribute an amount in <br />excess of Two Hundred One Dollars and Thirty Six Cents ($201.36) <br />per month per employee to provide the aforesaid coverage. <br /> <br />12.2 Dental <br /> <br /> Effective January 1, 1985, the City shall contribute an <br />amount necessary to provide dental care benefits under the <br />present Bankers Life Nebraska Plan for the individual employee <br />and eligible dependents; provided, however, that in no event <br />shall the City be obligated to contribute an amount in excess of <br />Thirty Eight Dollars and Thirty Five Cents ($38.35) per month <br />per employee to provide the aforesaid coverage. <br /> <br />12.3 <br /> <br />Life, Accidental Death & Dismemberment and Long-Term Disability <br /> <br /> The City shall contribute an amount necessary to provide life, <br />accidental death and dismemberment, and long-term disability insurance <br />under the City's present Standard Insurance Company Group policy <br />or agreed upon alternative policy for the individual employee; <br />provided, however, that in no event shall the City be obligated to <br />contribute an amount in excess of One Dollar and Fifty Cents ($1.50) <br />per One Hundred Dollars ($100.00) of monthly salary of the first <br />$2,250.00 of monthly salary not to exceed a total contribution of <br />Thirty Three Dollars and Seventy Five Cents ($33.75) per month per <br />employee. <br /> <br /> <br />
