Clearly <br />label/identify the specific program that includes the proposed project. <br /> <br />Most recent audit report. (if applicable) <br /> <br />PersonnelInformation (if applicable) <br /> <br />List of all paid positions and the salary of each position. If position is pan-time or <br />temporary, indicate percentage of time of on an annualized basis. Provide fringe benefit <br />information (retirement, healthplan, workers' compensation, employer's share of social <br />security, etc.) as a percentage of the total full-time and part-time salaries. <br /> <br />Identify contract services (bookkeeping, answering service, secretarial, etc.) and total <br />cost per contract on an annual basis. <br /> <br />Resolution, letter or other document providing evidence of Board/Organization <br />approval of application, and date approval was granted.
