ATTACHMENT3 CONTRACT ANNUAL SLURRY SEALING OF VARIOUS CITY STREETS PROJECT N0.075004 THIS CONTRACT is made and entered into this day of , 20_ by and between ("Contractor"), whose address is , and telephone number is and the CITY OF PLEASANTON, a municipal corporation ("City"). WITNESSETH: WHEREAS, the City has awarded to the Contractor a contract for ANNUAL SLURRY SEALING OF VARIOUS CITY STREETS - PROJECT NO. 075004. NOW, THEREFORE, in consideration of the mutual promises set forth herein, the parties agree as follows: 1. Work to be Performed. The work will consist of slurry sealing of thirty-eight (38) various City streets with approximately 845,000 squaze feet of black aggregate. Work shall consist of furnishing aggregate, latex emulsified asphalt, mineral filter and water properly proportioned, mixed and spread on pavement surface as specified in the Specifications and as directed by the Engineer. In addition, existing striping is to be removed and replaced prior to placing new striping and marking. Work shall consist of furnishing all labor, material, equipment, transportation and service necessary by these plans and Special Provisions. Said work is more particulazly shown in the following documents which aze on file with the Department of Public Works of the City and are incorporated herein by this reference: A. Approved Plans and Specifications entitled the ANNUAL SLURRY SEALING OF VARIOUS CITY STREETS, PROJECT NO. 075004 and addenda thereto, if any. B. Contract Change Orders approved by the City Engineer, done in accordance with the Standazd Specifications. C. The elements of the proposal submitted to the City by the Contractor, which the City has accepted.
