ATTACHMENT 3 CONTRACT RESURFACING OF VARIOUS CITY STREETS -PROJECT N0.085003 THIS CONTRACT is made and entered into this day of , 20 by and between and telephone number is corporation ("City"). ("Contractor"), whose address is , _ and the CITY OF PLEASANTON, a municipal WITNESSETH: WHEREAS, the City has awarded to the Contractor a contract for RESURFACING OF VARIOUS CITY STREETS - PROJECT NO. 085003. NOW, THEREFORE, in consideration of the mutual promises set forth herein, the parties agree as follows: 1. Work to be Performed. The work will generally consist of resurfacing approximately 1,150,000 square feet of various City streets with either Stress Absorbing Membrane Interlayer (SAMI) overlay construction method or with conventional overlay construction method (TYPE AC or Open Graded AC). The work generally consists of either placing the asphalt concrete over the existing pavement (conventional AC overlay) or placing the rubberized chip seal over existing pavement and placement of conventional asphalt concrete (SAMI). The work shall also include base repair, adjusting appurtenances to grade, installation of pavement markers, legends and striping, and traffic loops as required in the contract documents. The Contractor shall furnish all labor, materials, equipment, incidentals, transportation, traffic control and services necessary to complete the work in accordance with these Project Plans and Special Provisions. Said work is more particularly shown in the following documents which are on file with the Department of Public Works of the City and are incorporated herein by this reference: A. Approved Plans and Specifications entitled the RESURFACING OF VARIOUS CITY STREETS, PROJECT NO.085003 and addenda thereto, if any. B. Contract Change Orders approved by the City Engineer, done in accordance with the Standard Specifications.
