Invitation to Bid on "As Needed Services " <br />Section III <br />• Contractor's information: name, address, year established, former names, principals of <br />firm and professional status. <br />• Contractor's current workload, by listing all current contracts. <br />• Absence of any conflict of interest with this project. <br />• Statement of experience and qualifications. <br />• Listing of firm's equipment size and description, e.g., excavators, backhoe loaders, <br />truck and tools, etc. <br />• A sample of past similar projects include (client reference names and contact <br />information) and any additional information deemed necessary by firm to assist the <br />City in the selection process. <br />Submittal Process <br />Contractor's bid may be mailed or hand-delivered to: <br />Office of the City Clerk <br />City of Pleasanton Civic Center <br />123 Main Street <br />Pleasanton, CA 94566 <br />(Reference Bid # OSC-08-01 AD11~ <br />Mailed or hand-delivered proposals must be received by 2:OOpm on Friday, April 18, 2008. <br />Proposals received after the specified time will not be considered. <br />OTHER REQUIREMENTS <br />The rates shall remain firm during the entire first year term of the Contract. For the second or <br />subsequent years of the Contract term, the Contractor will be allowed with City approval and <br />upon written request to adjust prospectively pricing that will apply during that year. If <br />requested, pricing will be adjusted by the percentage change of the C.P.I. (Consumer Price <br />Index) for the San Francisco/Oakland/San Jose area for the year following the date of the <br />City's issuance of a written Notice to Proceed for the Contract, as evidence by the most recent <br />C.P.I. figures available to the City. <br />The term of this Agreement is from July lst, 2008 to June 30~', 2009. Upon written agreement <br />of both parties, the term of this Agreement may be extended for up to one (1) additional one- <br />year term. <br />The selected Contractor will be required to execute the City's standard form contract <br />agreement upon successfully negotiating a final scope and fee proposal with the City. A copy <br />of said agreement is enclosed as attachment 1 for review. <br />The selected Contractor and its sub-contractors shall be required to submit certificates of <br />worker's compensation insurance and liability insurance in accordance with the requirements <br />of the standard contract agreement. <br />14 <br />
