Required Attachments (this form to be completed as part of your <br />application). <br />o Only one (i) copy per Aged of each of the following is required, even with multiple <br />projects submitted. <br />o Applications without the following documents will n be reviewed for funding. <br />o Please label attachments A, B, C, etc. <br />Required Application Attachments Attached <br />A. Names and home addresses of Governing Board; identify current Board officers. <br />Home addresses are required. <br />• A minimum of one (1) Board member must be a Pleasanton resident. <br />B. Current total Agency operating budget, including revenue. ^ <br />• Clearly label/identify the program that includes the PROPOSED PROJECT. <br />Most recent audit report or tax return (if applicable). <br />C. Personnel Information (if applicable). <br />• List of all paid positions by classification and salary scale. If classification is <br />part-time or temporary, indicate percentage of time on an annualized basis. <br />Provide fringe benefit information (retirement, health plan, workers' <br />' ^ <br />s share of social security, etc.) as a percentage of the <br />compensation, employer <br />total full-time and part-time salaries. <br />• Identify contract services (bookkeeping, answering service, secretarial, etc.) and <br />total cost per contract on an annual basis. <br />D. Resolution, letter or other document providing evidence of Board/Organization <br />approval of application, and date approval was granted. <br />• Board/Organization approval may be pending. <br />E. Organization's certificate of insurance showing coverage for liability and workers' ^ <br />compensation. <br />F. Application Verification and Community of Character Declaration Signature ^ <br />Pa e. <br />G.
