This agreement is made and entered into on this date, December 1, 1988 <br />by and between Valley Memorial Hospital, hereinafter referred to as <br />"Hospital", and the fire department of the City of Pleasanton <br />hereinafter referred to as "City". <br /> <br /> WHEREAS the City is desirous of a California Emergency Medical <br />Technician 1-A recertification program offered onsite, and <br /> <br /> WHEREAS it is beneficial to the Hospital and its community to have <br />such a program, <br /> <br /> WHEREAS the Hospital meets the requirements of the certifying agency <br />and is willing to provide said recertification program, the par'ties <br />agree as follows: <br /> <br />Hospital Responsibilities <br /> <br />1. The Hospital will assure that the E/4T 1-A recertification program is <br /> duly accredited by the Alameda County E/4S authority. <br /> <br />2. The Hospital will recruit, select and reimburse the necessary faculty <br /> for participation in the program, as defined by the E/4S authority. <br /> <br />3. The Hospital will provide for the scheduling of sessions "on-shift" <br /> at the fire department training room to meet the needs detailed by <br /> the department's training officer. <br /> <br />4. The Hospital wil submit course rosters to the EMS authority in a <br /> timely fashion at the conclusion of the course that firefighters may <br /> complete the certification process. <br /> <br />The City <br /> <br />1. The City will designate the agency representative who will be <br /> responsible for registering students for classes, for securing the <br /> classroom facilities, and assuring ongoing communication with the <br /> primary instructor. <br /> <br />General Provisions <br /> <br />1. The Hospital will be compensated at a flat rate of $1360.00 per shift <br /> of firefighters enrolled. <br /> <br />2. Minimum shift enrollment is 10 persons; maximum shift enrollment is <br /> 30 persons. <br /> <br />3. Participants other than the firefighters regularly employed by the City <br /> may be invited on a space available basis at the discretion of the City <br /> representative. Those participants are responsible directly to the <br /> City for their enrollment fee. <br /> <br /> <br />
