EXHIBIT B <br />CONDITIONS OF APPROVAL <br />PCUP-167, Sherri Sweetnam, Jazz-N-Taps Dance Studio <br />3930 Valley Avenue <br />May 10, 2006 <br />1. The proposed use and its operation shall substantially conform to the plans and narrative, <br />Exhibit A, dated "Received Apri121, 2006", on file with the Planning Department, <br />except as modified by the following conditions. Minor alterations may be approved by <br />the Planning Director if found to be in substantial conformance with this approval. <br />2. If any element of the proposed use results in conflicts in regazd to parking, congestion, <br />noise, traffic, loitering, or other impacts, at the discretion of the Planning Director, this <br />use permit may be submitted to the Planning Commission for subsequent review at a <br />public hearing. If necessary, the Planning Commission may modify or add conditions <br />of approval to mitigate such impacts, or may revoke said use permit. <br />3. All Conditions of Approval of PUD-80-1 shall remain in full force and effect. <br />4. The hours of operation of the said use shall be from 9:00 a.m. to 9:00 p.m. Monday <br />through Friday, 9:00 a.m. to 3:00 p.m. on Saturdays, and closed Sunday. <br />5. If additional activities beyond that proposed in the written narrative and plans dated <br />"Received Apri121, 2006", on file with the Planning Department are desired, prior City <br />review and approval shall be required. Such modifications shall be submitted to the <br />Planning Director, who shall approve requested minor modifications or refer the matter <br />to the Planning Commission if determined to be substantial. <br />6. There shall be no outdoor storage. <br />The applicant shall maintain the area surrounding the tenant space in a clean and orderly <br />manner at all times. <br />8. If signing for the business is desired, a sign application shall be submitted to the Planning <br />Director for consideration under a sepazate application. <br />9. At no time shall balloons, banners, pennants, or other attention-getting devices be utilized <br />on the site except as allowed by Section 18.096.060 (K) of the Zoning Ordinance for <br />grand openings or by Section 18.116.040 of the Zoning Ordinance if approved by <br />temporary conditional use permit as part of a decorating plan in conjunction with <br />shopping center promotional events. At no time shall spot lighting be used in <br />conjunction with grand openings and/or promotional events. <br />10. No changes to the exterior of the building shall be made without prior approval from the <br />Planning Department. <br />
