EXHIBIT B <br />CONDITIONS OF APPROVAL <br />PCUP-151, Mario Montalvo, Vision Food Service <br />5776 Sonoma Drive, Suite B <br />Apri126, 2006 <br />The location and operation of the proposed use shall conform substantially to Exhibit <br />"A" (the site/floor plan, and details of operation), dated "Received, March 21, 2006", <br />on file with the Planning Department, except as modified by the following conditions <br />of approval. Minor changes to the approved operation and schedule may be approved <br />by the Planning Director if found to be in substantial compliance with the proposal. <br />2. Additional hours and activities beyond what was stated in the applicant's statement of <br />operation, dated "Received, March 21, 2006", on file in the Planning Department may <br />be approved by the Planning Director. The Planning Director may also refer the <br />matter to the Planning Commission if the proposed changes would be significant. <br />If operation of this use permit results in conflicts pertaining to parking, noise, <br />traffic/circulation, or other factors, at the discretion of the Planning Director, this <br />conditional use permit may be submitted to the Planning Commission for their <br />subsequent review at a public hearing. If necessary, the Planning Commission may <br />modify or add conditions of approval to mitigate such impacts, or may revoke the <br />said conditional use permit approval. <br />4. The storage and minor repair and maintenance of the vending machine equipment <br />approved to be stored in the building shall be conducted entirely within the building <br />at all times. <br />5. A building permit, if required, shall be obtained prior to commencement of any tenant <br />improvement work. <br />6. The applicant shall contact the Fire Marshal to conform to Fire Code requirements, if <br />applicable. <br />7. No changes to the exterior of the building shall be made without prior approval from <br />the Planning Department. <br />8. Any signage shall conform to the City's signage standards for the I-P District. <br />9. This conditional use permit approval will lapse one (1) year from the effective date of <br />approval unless the applicant receives a business license. <br />{end} <br />
