The mylars submitted shall contain the following, where <br />applicable: <br />(1) Project name(s) <br />(2) Project number(2) <br />(3) North arrow <br />(4) Graphic scale <br />(5) A written scale showing the actual scale of the <br />reduced mylar copy <br />(6) Name, address, and telephone number of the firm and <br />the name of the individual contact that provided <br />the mylar copy <br />11.
