Res. No. 2150 EXHSBIT "A" UP-82-2 <br />1. That this use permit is granted only for the approximately 1870 <br />sq. ft. portion of the building located at 4301 Valley Avenue, <br />formerly occupied by the Sports Den. <br />2. That the applicant shall provide additional lighting in the parking <br />area of the subject property as determined necessary by the <br />Police Department. This lighting shall be installed at the <br />applicant's expense and shall be for the purpose of allowing proper <br />surveillance of the subject property by the Police Department. <br />3. That at least one adult supervisor shall be on duty on the premises <br />at all times. After 6:OOpm there shall be at least two adult <br />supervisors on duty on the premises on Fridays, Saturdays and <br />preholidays. <br />4. That if determined necessary by the Chief of Police, the applicant <br />shall employ a security guard to patrol the parking area of the subject <br />property in order to discourage loitering. <br />5. That this use permit is granted for a period of five (51 years. <br />The use permit will be reviewed in six months by the Planning <br />- Commission to determine if additional conditions of operations are <br />necessary. <br />6. That the applicant shall install bicycle racks in at least two <br />parking spaces adjacent to the entrance to the game room. The layout <br />of the bike rack area shall be subject to approval by the Planning <br />Division. All bikes shall be stored in these racks at all times. <br />7. The hours of operation of the game room shall be as follows: <br />Monday through Thursday 10:-00am to 10:OOpm; Friday and Saturday <br />10:00am to Midnight; and Sunday 12:00 Noon to 10:00pm. Adjustments <br />to the hours of operation will require approval of the Director <br />of Planning and Community Development. <br />8. That the applicant be aware that the use of the premises as a game <br />room will change the occupancy type of the building from B2 to A3, <br />necessitating at least two exit doors, panic hardware on all doors, <br />signs, etc. and that the applicant should consult with the City <br />Fire Marshal for further information and requirements. <br />3/10/82 <br />
