6.24.060 <br />be licensed, at the massage establish- <br />ment, up to a maximum of one million <br />dollars ($1,000,000.00); this require- <br />ment can be satisfied by malpractice <br />insurance being provided in the name <br />of individual massage technicians. <br />22. Written authorization for the <br />City, its agents, and employees to seek <br />information and conduct an investiga- <br />tion into the truth of the statements set <br />forth in the application and into the <br />background of the applicant and the <br />responsible managing officer/employee <br />of the massage establishment. <br />23. Such other identification and <br />information shall be provided as re- <br />quired by the Police Chief, necessary to <br />discover the truth of the matters speci- <br />fied and required in the application. <br />(Ord. 1727 § 2, 1997) <br />6.24.070 Massage Technician Per- <br />mit Application And Fee: <br />A. Each applicant for a massage <br />technician permit shall file a written <br />application with the Police Chief on a <br />form provided by the Police Chief. <br />B. Each application shall be accom- <br />panied by a nonrefundable fee, in an <br />amount established by resolution of the <br />City Council. The application fee shall <br />be used to defray, in part, the costs of <br />the investigation and report, and is not <br />made in lieu of any other fees or taxes <br />required under this Code. Separate <br />payment shall be made, payable by <br />money order to Department of Justice, <br />to process fingerprints as required by <br />subsection 6.24.060019 of this Chapter. <br />A copy of the receipt for the nonre- <br />fundable fee shall accompany the appli- <br />cation. <br />C. Each applicant shall submit the <br />following information and/or docu- <br />ment(s) under penalty of perjury: <br />1. As required by subsection <br />6.24.OSOA of this Chapter, an original <br />diploma, certificate of graduation or <br />other written proof, including certified <br />sealed transcripts, acceptable to the <br />Police Chief, that the applicant has met <br />the education requirements set forth, or <br />written proof that the applicant has <br />successfully completed the National <br />Certification Board for Therapeutic <br />Massage and Bodywork examination. <br />2. As required by subsection <br />6.24.OSOB of this Chapter, written <br />proof that the applicant is a member, in <br />good standing, of a State or national <br />professional association devoted to the <br />massage specialty and therapeutic en- <br />hancement approach. <br />3. All information required by sub- <br />section 6.24.0600 of this Chapter, ex- <br />cept the following: <br />a. Each applicant shall provide only <br />the name, street address, and telephone <br />number of each other business in which <br />the applicant has been employed within <br />the past seven (7) years along with the <br />dates of employment, positions held by <br />the applicant, and a contact person at <br />each location to comply with subsection <br />6.20.06005 of this Chapter; <br />b. Each applicant need not provide <br />information as required in subsection <br />6.24.0600 12, C 13, C 15, C 16, C 17, and <br />020 of this Chapter; and <br />c. Each applicant shall provide writ- <br />ten authorization for the City, its <br />(Pleasanton 5-98) 1 60 <br />
