6.24.060 be licensed, at the massage establish- ment, up to a maximum of one million dollars ($1,000,000.00); this require- ment can be satisfied by malpractice insurance being provided in the name of individual massage technicians. 22. Written authorization for the City, its agents, and employees to seek information and conduct an investiga- tion into the truth of the statements set forth in the application and into the background of the applicant and the responsible managing officer/employee of the massage establishment. 23. Such other identification and information shall be provided as re- quired by the Police Chief, necessary to discover the truth of the matters speci- fied and required in the application. (Ord. 1727 § 2, 1997) 6.24.070 Massage Technician Per- mit Application And Fee: A. Each applicant for a massage technician permit shall file a written application with the Police Chief on a form provided by the Police Chief. B. Each application shall be accom- panied by a nonrefundable fee, in an amount established by resolution of the City Council. The application fee shall be used to defray, in part, the costs of the investigation and report, and is not made in lieu of any other fees or taxes required under this Code. Separate payment shall be made, payable by money order to Department of Justice, to process fingerprints as required by subsection 6.24.060019 of this Chapter. A copy of the receipt for the nonre- fundable fee shall accompany the appli- cation. C. Each applicant shall submit the following information and/or docu- ment(s) under penalty of perjury: 1. As required by subsection 6.24.OSOA of this Chapter, an original diploma, certificate of graduation or other written proof, including certified sealed transcripts, acceptable to the Police Chief, that the applicant has met the education requirements set forth, or written proof that the applicant has successfully completed the National Certification Board for Therapeutic Massage and Bodywork examination. 2. As required by subsection 6.24.OSOB of this Chapter, written proof that the applicant is a member, in good standing, of a State or national professional association devoted to the massage specialty and therapeutic en- hancement approach. 3. All information required by sub- section 6.24.0600 of this Chapter, ex- cept the following: a. Each applicant shall provide only the name, street address, and telephone number of each other business in which the applicant has been employed within the past seven (7) years along with the dates of employment, positions held by the applicant, and a contact person at each location to comply with subsection 6.20.06005 of this Chapter; b. Each applicant need not provide information as required in subsection 6.24.0600 12, C 13, C 15, C 16, C 17, and 020 of this Chapter; and c.
