6.24.060 <br />city, county, territory, or state; the date <br />of issuance of such permit or license; <br />whether or not the permit or license is <br />still in effect; if the permit or license is <br />no longer in effect, whether or not it <br />was revoked or suspended, and if so, <br />the reason(s) therefor. The name and <br />location of the jurisdiction or agency <br />which suspended or revoked such li- <br />cense, certificate, permit, or other au- <br />thorization shall also be included. <br />7. The name and street address of <br />any other massage business operated or <br />managed by the applicant during the <br />last seven (7) years. <br />8. A statement whether the applicant <br />intends to personally provide massage <br />services at the business. <br />9. Except as provided in subsection <br />6.24.OSOC of this Chapter, an original <br />diploma, certificate of graduation, or <br />other written proof, including certified <br />sealed transcripts, acceptable to the <br />Police Chief or written proof of the <br />applicant's successful completion of the <br />National Certification Board for Thera- <br />peutic Massage and Bodywork exam- <br />ination showing that the applicant has <br />met the requirements of subsection <br />6.24.OSOA of this Chapter. <br />10. Except as provided in subsection <br />6.24.OSOC of this Chapter, written <br />proof that the applicant is a member, in <br />good standing, of a State or national <br />professional association devoted to the <br />massage specialty and therapeutic en- <br />hancement approach as required by <br />subsection 6.24.OSOB of this Chapter. <br />11. A statement that within the seven <br />(7) years preceding submission of the <br />application, whether the applicant has <br />been convicted or entered a plea of <br />nolo contendere in a court of competent <br />jurisdiction of any of the offenses listed <br />below. For the purposes of this subsec- <br />tion, "applicant" means the following: <br />an individual if the applicant is an indi- <br />vidual; any officers, directors, stock- <br />holders holding more than five percent <br />(5%) of the stock of the corporation, or <br />the managing responsible officer, if the <br />applicant is a corporation; and a partner <br />or the managing responsible officer, if <br />the applicant is a partnership. <br />a. Conduct which requires registra- <br />tion under California Penal Code sec- <br />tion 290. <br />b. Conduct which is in violation of <br />California Penal Code section 220, <br />245.3, 245.5, 261, 264.1, 266(e), <br />266(h), 266(1), 314, 315, 316, 318, <br />647(a), 647(b), or 647(d). <br />c. Crimes that are designated in Cali- <br />fornia Government Code section <br />51032(b). <br />d. Any other crime involving dishon- <br />esty, fraud, deceit, violence, or moral <br />turpitude. <br />e. Conspiracy or attempt to commit <br />any of the aforedesignated offenses. <br />Convictions that have been expunged <br />and convictions under the laws of other <br />jurisdictions which proscribe the same <br />or similar conduct as the afore- <br />designated crimes shall be reported. <br />Whether the applicant has been sub- <br />jected to a permanent injunction against <br />the conducting or maintaining of a nui- <br />sance pursuant to sections 11225 <br />through 11235 of the California Penal <br />Code, or any similar provisions of law <br />in a jurisdiction outside the State. <br />(Pleasanton 5-98) 158 <br />
