The undersigned will, upon request of Owner, furnish certificates <br /> of insurance evidencing the above insurance coverages. <br /> <br /> CONTRACTOR SAFETY <br /> <br />CONTRACTORS and/or SUBCONTRACTORS <br /> <br />This is to advise you and your employees that any work conducted <br />on our, KAISER SAND & GRAVEL COMPANY, properties must comply with <br />local, state and federal rules, regulations, safety orders, and <br />laws which are included in Kaiser Sand & Gravel Company programs. <br /> <br />Your organization will be required to comply primarily with <br />California Safety & Health Act (CAL-OSHA) in our operations such <br />as Readymix and Asphaltic Concrete plants. You will comply with <br />the Federal Mine Safety & Health Administration (MSHA) rules in <br />mining and aggregate producing operations, and related shops. On <br />Kaiser Sand & Gravel Company aggregate mining and producing <br />properties, you will be required to obtain a Contractor Mine <br />Identification Number for your organization from MSHA. Should an <br />MSHA compliance officer conduct an inspection, your company personnel <br />and/or equipment may be cited for unsafe condition or acts. <br /> <br />Mine identification numbers may be obtained by calling the local <br />office of the Mine Safety and Health Administration (MSHA). <br /> <br />All safety equipment required and observance of safety regulations <br />is the responsibility of the Contractor/Subcontractor. <br /> <br />ACCEPTED AND AGREE TO THIS DAY OF <br /> <br /> COMPANY <br /> <br /> Signature <br /> <br /> Title <br /> <br />07/87 <br />lw <br /> <br /> <br />
