CONTRACT NO. 0HS-573-89 <br /> <br /> AGREEMENT FOR EMPLOYEE <br /> ASSISTANCE PROGRAM~S~VICES ~ <br /> This Agreement, dated this Li~i,-day of-M~q~, 1989, by and <br />between THE CITY OF PLEASANTON (hereinafter referred to as <br />"Sponsor") and Occupational Health Services, Inc., a California <br />corporation (hereinafter referred to as "Contractor"). <br /> <br /> RECITALS <br /> <br /> Contractor operates an Employee Assistance Program that <br />provides counseling services to individuals with various personal, <br />medical-behavioral problems and is licensed as a specialized health <br />care service plan under the Knox-Keene Health Care Service Plan Act <br />of 1975. <br /> <br /> Sponsor wishes to make the Program available to Subscribers <br />and their dependents. <br /> <br /> Now, therefore, the parties mutually agree as follows: <br /> <br /> I. DEFINITIONS <br /> <br /> For purposes of this agreement and any other document <br /> <br />executed or provided in connection with this Agreement, the <br />following terms shall have the meanings given them in this Section <br />unless the context requires otherwise: <br /> <br /> A. Employee Assistance Proqram thereinafter referred to <br />as the "Proqram"): An organized procedure for the provision of <br />assistance and counseling to persons experiencing personal, <br />medical-behavioral problems related to family conflict, drug or <br />alcohol abuse, stress, marital discord, finances, and other matters, <br />and for the provision of training, consultation and other management <br />services relating to the effective utilization of the Program by <br /> <br /> - 1 - <br /> <br /> <br />
