Applicant certifies that it has submitted a satisfactory independent fiscal audit with required compli- ance statement to M'I'C and to the State Controller (Pub. Util. Code §99245 and 21 Cal. Code of Regs. §6664) covering FY 2005-06. Applicant also assures that a fiscal audit pursuant to the cited requirement will be completed covering FY 2005-06. X B. Applicant certifies that it has submitted a State Controller's report in conformance with the Uniform System of Accounts and Records to MTC and to the State Controller pursuant to Pub. Util. Code §99243, for FY 2005-06. Applicant further assures that a State Controller's Report will be completed covering FY 2005-06. X C. Applicant certifies that it is regularly participating in the activities of its county's Paratransit Coordinating Council (PCC). (See MTC Resolution No. 1209, revised). D. (FOR STA FUNDS) Applicant for S"fA funds certifies that it is making full use of federal funds available under the federal surface transportation authorizing legislation. E. All applicants must certify the following: a. if applicant is an operator and offers reduced fares to senior citizens and disabled persons, applicant will honor either the federal Medicare identification card or the California Department of Motor Vehicles senior citizen identification card, as well as the Regional Transit Connection Discount Card; and X b. if applicant is an operator and offers reduced fares to senior citizens, applicant will also offer the identical reduced fare to disabled patrons. F. (APPLIES TO ARTICLE 8 RECIPIEN"fS) Applicant that is a municipal operator (as defined under Pub. Util. Code §99209) certities that it has submitted a copy of the California Highway Patrol (ChP) certitication, which was issued within the last 13 months, indicating compliance with Cal. Veh. Code § 1808.1 (CHP "pull notice system and periodic reports"). (Pub. Util. Code §99251). G. Applicant certifies that it does not require that a person be a resident of the applicant's service area in order to quality for service. H. (FOR STA FUNDS) Applicant for STA funds certities that it has current SB 602 "joint tare revenue sharing agreements" in place with transit operators in the MTC region with which its service connects, and that it has submitted copies of all such current agreements to M"I'C. L (FOR STA FUNDS) Applicant for STA funds certifies that it complies with MTC's Transit Coordination Implementation Plan (MTC Resolution No. 3055, revised). (Public Util. Code §99314.5(c) and §99314.7). The undersigned attests that the certifications indicated above are true and correct, and further attests to the applicant's intent to comply with the indicated certifications.
