A growing need for in-home workers in our communits- is ~ell <br />documented by our agenc~~. We have found that one of the <br />primary needs of frail elders is for choreworker services <br />order to remain independent. <br /> <br />Hours of Service Provided <br /> <br />It takes approximately 8 hours to place a care provider/house <br />cleaner in an elder's home. <br /> <br />Worker: <br /> <br />Recruit worker <br />Interview and screen <br />Check references <br />2-3 hrs. training <br /> <br />Eider: <br /> <br />Intake for service <br />Home visit (an assessment of elder and needs) <br />Provide information on hiring help <br />Referral of possible workers <br />Monitoring progress of worker <br /> <br />Geriatric Resources-Case Management: <br /> <br />Geriatric Resources is the case management component of the <br />Senior Support Program and began in 1986. <br /> <br />Case Management is an individualized service provided by a <br />case manager acting in the role of a coordinator to ensure <br />that the various needs of a frail eider are assessed and met. <br />It consists of a face-to-face discussion resulting in the <br />determination of needs, the development of the care plan and <br />the on-going monitoring of the plan. Case management implies <br />a collaborative and cooperative working relationship with the <br />individual and those relating to him/her, and with the <br />crucial problems of his/her life. Case management services <br />imply help in gaining access to resources, help in using <br />those resources as well as help in securing rights and <br />entitlements for independent living. <br /> <br />Hours of Service Provided <br /> <br />Case Management: Per client-initial work-up-S hours. <br /> <br />Intake for service <br />Home visit (an assessment of elder and needs) <br />Care Plan and Implementation ~,f services <br />Mon£toring-continues on un on-going basis. Hours <br /> depending on severity of the problems. <br /> <br /> <br />
