12.2 Dental <br /> <br /> Effective March 1, 1990, the City shall contribute an <br /> amount necessary to provide dental care benefits under <br /> the present Bankers Life Nebraska Plan for the individual <br /> employee and eligible dependents; provided, however, that <br /> in no event shall the City be obligated to contribute an <br /> amount in excess of Forty Eight Dollars(S48.00) per month <br /> per employee to provide the aforesaid coverage. <br /> Effective May 1, 1991, the City shall increase the <br /> maximum orthodontia coverage under the Bankers Life <br /> Nebraska Plan from seven hundred fifty dollars ($750.00) <br /> to one thousand dollars ($1,000.00). <br /> <br /> 12.3 Lonq Term Disability <br /> <br /> Effective January 1, 1989, the City's maximum monthly <br /> contribution shall be increased to One Dollar and Eighty <br /> Four cents ($1.84) per One Hundred Dollars ($100.00) of <br /> monthly salary of the first $2,250.00 of monthly salary <br /> not to exceed a total contribution of Forty One Dollars <br /> and Thirty Nine cents ($41.39) per month per employee. <br /> <br /> 12.4 Life, Accidental Death and Dismemberment Insuranc~ <br /> <br /> Effective March 1, 1991 the city shall increase the <br /> present employee $25,000 life and $25,000 accidental <br /> death and dismemberment insurance coverage to $35,000 <br /> life and $35,000 accidental death and dismemberment <br /> insurance coverage. The monthly premium for life <br /> insurance coverage shall be paid by the City. <br /> <br /> Section 13. Retirement <br /> <br /> 13.1 Payment of Employee Contribution to PERS <br /> <br /> Effective January 1, 1988, the City shall pay the <br /> employee's contribution to the Public Employees' <br /> Retirement System (PERS), to include the "Single Highest <br /> Year" option. The amount of said City payment shall <br /> equal but not exceed nine percent (9%) of the employee's <br /> compensation subject to retirement contributions. <br /> <br /> The city shall allow an employee one time to convert <br /> from city payment of the employee's nine percent (9%) <br /> PERS contribution to employee payment of the nine <br /> percent (9%) PERS contribution. The employee's base <br /> monthly salary shall be increased nine percent (9%). <br /> <br />Section 14. Uniforms <br /> <br /> The City shall provide all necessary safety and <br /> protection clothing, as defined by the City Manager. The <br /> City shall provide fifty percent (50%) of the initial <br /> purchase price of uniforms for those employees required <br /> to wear a prescribed uniform. <br /> <br />Memorandum of Understanding <br />Page 14 <br /> <br /> <br />
