Any employee returning from medical leave shall be <br /> required to provide a physician's release to return to <br /> duty. <br /> <br /> 11.3 Jury'Leave <br /> <br /> Every employee of the City who is called or required to <br /> serve as a juror shall be entitled to leave from his/her <br /> duties with the City during the period of such service or <br /> while necessarily being present in court as a result of <br /> such call. The employee shall be paid the difference <br /> between his/her full salary and any payment received by <br /> him/her, except travel pay, for such duty. <br /> <br /> 11.4 Leaves Resultinq from Subpoen~ <br /> <br /> Leave of absence with pay shall be granted to a person <br /> while going to and from court and answering a subpoena as <br /> a witness. The employee shall be paid the difference <br /> between his/her full salary and any payment received by <br /> him/her, except travel pay, for such duty. <br /> <br /> 11.5 Military Leave <br /> <br /> Military leave shall be granted in accordance with the <br /> provisions of law. All employees entitled to military <br /> leave shall give the Appointing Authority an opportunity <br /> within the limits of military regulations to determine <br /> when such leave shall be taken. <br /> <br /> 11.6 Return from Leave <br /> <br /> Upon the expiration of a regularly approved leave, the <br /> employee shall be reinstated in the position held at the <br /> time the leave was granted. Failure on the part of an <br /> the employee on leave to report promptly at its <br /> expiration shall be cause for discipline up to and <br /> including dismissal. <br /> <br />Section 12. Health and Welfare Coverage <br /> <br />12.1 Hospital and Medical - Active Employees <br /> <br /> Effective March 1, 1990, the City shall contribute an <br /> amount necessary to provide hospital and medical care <br /> benefits under either the present Blue Cross Plan, Kaiser <br /> S-1 Plan or, Foundation Health Plan I for the individual <br /> employee and a portion of the amount necessary to provide <br /> coverage for eligible dependents; provided, however, in <br /> no event shall the City be obligated to contribute a <br /> monthly amount in excess of the monthly premium for the <br /> Kaiser S-1 Health Plan full family coverage. <br /> <br />Memorandum of Understanding <br />Page 12 <br /> <br /> <br />
