EXHIBIT B <br /> <br />TERMS AND CONDITIONS OF .PAYMENT <br /> <br />1. This attachment sets forth the terms and conditions by <br />which payment will be made by the City to the Contractor for <br />expenses incurred pursuant to the duties and services listed <br />in Exhibit A of this contract. <br /> <br />2. Except as noted below, all payments to the Contractor <br />shall be made on a reimbursement basis (i.e. Contractor shall <br />seek reimbursement from the City after expenses have already <br />been incurred). <br /> <br />3. Ail requests for reimbursement shall be submitted in the <br />form of an invoice or letter. Requests for reimbursement <br />shall be dated and signed by 1) the Executive Director, and <br />2) the Contractor's accountant or a Board member. <br /> <br />4. Requests for reimbursement shall be accompanied by <br />supporting documentation which gives evidence of the expenses <br />incurred by the Contractor (i.e. valid receipts and/or copies <br />of cancelled checks made to the sub-contractor). Said <br />documentation shall include sufficient detail to enable <br />proper account coding by the City Finance Department. The <br />City will not make direct payment to the Contractor if <br />evidence of payment to the sub-contractor is not provided. <br />However, the City may issue payment directly to the sub- <br />contractor~with proper documentation. <br /> <br />5. Whenever possible, requests for reimbursement shall be <br />compiled by the Contractor for submittal on a monthly basis <br />to the City. While the City will try to accommodate unusual <br />or emergency requests for reimbursement whenever possible, <br />the City will bear no responsibility for any consequences <br />suffered by the Contractor through adherence to a monthly <br />system of handling reimbursements. <br /> <br />6. The Contractor shall maintain copies of all financial <br />records related to this contract. The information shall be <br />available at all times for review by the City and/or any <br />other governmental entity pursuant to the terms of this <br />contract. <br /> <br />(hcdexhb. ltr) sfe <br />6/1/89 <br /> <br /> <br />
