I EXHIBIT B CONTRACT SLURRY SEALING OF VARIOUS CITY STREETS PROJECT NO, OSS004 THIS CONTRACT is made and entered into this _ day of , 20 by and between , ("Contractor"), whose address is , and telephone number is and the CITY OF PLEASANTON, a municipal corporation ("City"). WITNESSETH: WHEREAS, the City has awarded to the Contractor a contract for Slurry Sealing of Various City Streets, Project No. 055004. NOW, THEREFORE, in consideration of the mutual promises set forth herein, the parties agree as follows: 1. Work to be Performed. The work will consist of slurry sealing of eleven (11) various City streets with approximately 700,000 square feet of black aggregate. Work shall consist of furnishing aggregate, latex emulsified asphalt, mineral filter and water properly proportioned, mixed and spread on pavement surface as specified in the Specifications and as directed by the Engineer. In addition, existing striping is to be removed and replaced prior to placing new striping and marking. Said work is more particularly shown in the following documents which are on file with the Department of Public Works of the City and are incorporated herein by this reference: A. Plans and Specifications entitled the Slurry Sealing of Various City Streets, Project No. 055004 and addenda thereto, if any. B. Contract Change Orders approved by the City Engineer, done in accordance with the Standard Specifications. C. The proposal submitted to the City by the Contractor. 2. Compensation. The City shall pay the Contractor for work actually performed at the unit prices set out in the Contractor's proposal to the City. The quantities of work stated therein are estimates only; actual quantities will be measured for payment in accordance with the specifications.
