In the case <br />where the eligible employee is deceased, the amount the city is <br />obligated to pay shall be reducedby 1/2 (one-half). If the spouse <br />remarries, the obligation by the city shall terminate. <br /> <br />6.3 Dental Insurance <br /> The city at its expense shall provide management and <br /> confidential employees and- their eligible dependents <br /> enrollment in the present Ameritis Bankers Life Nebraska <br /> dental plan. Plan benefits shall include a maximum lifetime <br /> orthodontia benefit of $1,500. <br /> <br /> 6.4 Life Insurance. <br /> For the purpose of providing life and accidental death and <br /> ~ismemberment insurance to management and confidential <br /> employees, the city shall provide at its expense coverage <br /> equal to twice the employee's annual salaz7 to a maximum <br /> coverage of one hundred thousand dollars ($100,000). <br /> <br /> 6.5 Lonq-Term Disability Insurance, <br /> The city shall provide at its expense management and <br /> confidential employees long term disability insurance <br /> coverage.
